Objective: In recent years found that irritable bowel syndrome (with irritable bowel syndrome, IBS), inflammatory and immune abnormalities in the intestinal mucosa of patients, the cytokine secretion disorders play a role in the abnormal immune response baking, but its exact pathogenesis is not yet clear. To further clarify the infection, the role of inflammation in the pathogenesis of IBS, this study by detecting IBS patients, especially post-infection IBS (Irritable Bowel syndrome post-infection, pIBS) plasma of patients with colorectal interleukin (interleukjn, IL) - 6, IL-13, IL-18 of the expression of inflammatory cytokines IBS, especially the role of infection in the pathogenesis of IBS, the study will explore the pathogenesis and clinical treatment of IBS to provide new ideas and theories basis. Research object and method random sample of 67 cases of our hospital of Gastroenterology door Jane IBS patients, 25 males and 42 females, average age 42.3 ± 10.86 years old, are in line with the Rome Ⅲ criteria diagnosis. 50 cases of patients with diarrhea-predominant IBS, constipation-predominant patients in 17 cases. Patients with diarrhea-predominant in 23 cases of acute gastroenteritis or dysentery history of patients with constipation-more than a history of four cases. Normal control group of 20 patients. All subjects by ELISA determination of plasma IL-6, IL-13, IL-18 level; conventional colonoscopy, take rectum, ileocecal mucosa using immunohistochemical method for detection of IL-6, IL-13, IL 18 expression. Statistical analysis of the indicators in the diarrhea-predominant IBS, constipation-predominant IBS, and the normal control group, plasma test results TTEST test, mucous membrane test results using the chi-square test; infection IBS, IBS patients with non-infectious indicators ANVOA inspection. Results, the expression of inflammatory cytokines in the intestinal mucosa of patients with IBS: proinflammatory cytokines IL-6, IL-18, respectively, in patients with diarrhea-predominant IBS number of positive expression of 32 (64%), 35 (70%), significantly higher than the normal control group (P <0.05), the positive expression in patients with constipation-7 (41%), 9 (52%) is also higher than that in the control group, but the difference was not statistically significant (P> 0.05). For the anti-inflammatory cytokine IL-13 in the intestinal mucosa expression of diarrhea-predominant and constipation-predominant IBS patients have increasing trend compared with the control group, the difference was not significant sex (P> 0.05). Expression of inflammatory cytokines in the plasma of patients with IBS: Compared with the control group, the proinflammatory cytokines IL-6, IL-18 plasma levels significantly higher plasma levels in patients with diarrhea-is 116.29 ± 43.26pg / ml, 5.62 ± 4.0l pg / ml, increased more significantly (P <0.05), although the patients with constipation-rise, but the difference was not significant (P> 0.05). The anti-inflammatory cytokine IL-13 plasma levels decreased to 50.35 ± 7.69pg/ml, but no significant difference (P> 0.05). 3, infection factors cytokines in patients with diarrhea-predominant IBS: infection IBS and non-infected blood plasma and intestinal mucosa of patients with IBS IL-6, IL-13, IL-18 and control group was statistically analyze found IL -6, IL-18 plasma levels of infection IBS patients was 117.26 ± 45.06pg/ml, 5.43 ± 4.28pg/ml the mucosal expression positive rate was 60.86%, 56.25%, were higher than non-infected patients with IBS, but the difference was not statistically significant (P> 0.05). Infection IBS of plasma and mucosal IL-6, IL-18 levels higher, the difference was significant (P <0.05), non-infectious IBS patients plasma and mucosal IL-6, IL-18 level was also higher than that of the control group, but the difference was no statistically significant (P> 0.05). IL-13 after infection IBS and non-infectious IBS patients the plasma decreased expression, is 49.63 ± 7.75 pg / ml, 50.61 ± 7.66 pg / ml, in the mucosal expression positive rates were 47.82%, 48.14%, but the difference compared with the control group There was no statistically significant (P> 0.05). Conclusion pro-inflammatory cytokines are increased plasma and intestinal mucosa in patients with IBS expression is more obvious, especially in patients with diarrhea-predominant IBS expression, while the anti-inflammatory cytokine levels did not change significantly. Prompt IBS patients not only in the intestinal mucosa, and there is the expression of pro-inflammatory cytokines and anti-inflammatory cytokines imbalance in the average plasma water. The plasma expression of inflammatory cytokines also licensed as a clinical detection methods for diagnosis of IBS the new auxiliary diagnostic measures and theoretical basis. 2 after infection IBS patients compared with non-infected patients with IBS to promote the expression of inflammatory cytokines compared with control group increased more significantly, further confirmed infection caused by inflammatory reactions involved in the pathogenesis of IBS, and affect the balance of cytokines adjustment.
