Abstract


1. The diagnosis and treatment review of78cases of bullous pemphigoidObjective: Bullous pemphigoid is a common clinically autoimmune hydroa, andits pathogenesis is complex,and have it is own chavacteristics in clinical manitestationand labovatag diagnosis. we made the clinical review of diagnosis and treatment in78patients with pemphigoid in our hospital to provide a theoretical basis for clinicaldiagnosis and treatment.Methods: We collected78case history medical records of hospitalized patients，who had been diagnosised and treated in dalian Hospital of Dermatology ward duringJanuary2007to January2012. We made a registration about age of onset, duration, skinlesions feature, the severity of the disease, histopathology (immune pathology), whethermerger of the nervous system symptoms, hospital stays, and treatment protocols，andconducted a retrospective analysis.Result: This group had78cases, including41males (52.56%) and37females(47.43%). Their age range from50years old to94years old, average age was74.01±9.49, Typing on the basis of distribution of lesion area，28patients were Mild,41casesof patients were moderate and9patients were severe. Among them，13patients withmucosal damage, accounting for16.6%, and the severity of the lesions of the diseaseand treatment time were proportional to mucosal damage. The concordance rate ofclinical manifestations, pathological examination and immunofluorescence was69.23%,15patients combined witf nervous system diseases accounted for18%. The systemapplication of corticosteroids is the primary means of which treatment, equivalent toprednisone0.5-1.2mg/(kg d) which can control the majority of the patient’scondition. 2.The expression of CXCR5in lesional skin of BP and its clinicalsignificanceObjective:To find out the effect of helper T lymphocytes in the pathogenesis ofPemphigus vulgaris through the detection of chemokine receptor-5(CXCR5)onfollicular helper T cells (Tfh)in pemphigoid lesions.Method:we chose16pemphigoid tissues and10normal healthy tissues toexamined the expression of chemokine receptor-5(CXCR5)by immunohistochemicalprocedures.Result:CXCR5is expressed inthe basement membrane of the normalhealthy skin, while is irregularly diffused in basement membrane and stratumspinosum and have significance divergence(P<0.05).Conclusion:CXCR5is irregularly diffused in basement membrane and stratumspinosum of pemphigoid lessions with positive expression rate of950%,which isdifferent with normal skin.And we speculate CXCR5Plays a importment role in thepathogenesis of pemphigoid.Conclusion1. BP mainly occurs in the elderly, but young patients with more severe skineruptions than older patients.2. Histopathology and immunofluorescence are the main method for diagnosingBP.3. Bullous pemphigoid can combined with.nervous sgstem disease.4.Glucocorticoid is main treatment for the control of the disease, adoptingglucocorticoids shock therapy for Ineffective cases of conventional treatment5.For the intensine patients with bad carative effect or taboo a lacocrticoul and ifnecessary, shock therapy with intravenous gammaglobulin (IVIG) is selected.6. CXCR5is expressed inthe basement membrane of the normal healthyskin,while is irregularly diffused in basement membrane and stratum spinosum.7. CXCR5Plays a importment role in the pathogenesis of pemphigoid.
