Abstract


Objective To study the characteristics of lung function in patients with nasal polyps, nasal polyps by observing airway hyperresponsiveness in patients with asthma as well as the incidence of abnormal lung function, to understand the severity of nasal polyps and lower respiratory disease if there is a correlation. Carried out simultaneously with nasal polyps and allergic lower respiratory tract disease correlation. Patients with nasal polyps to explore the upper and lower respiratory tract correlation for lower respiratory tract disease prevention and therapy. Materials and Methods: a study object: The selected cases were from March 2010 to June 2010, Shandong University Qilu Hospital ENT ambulatory surgery patients were 56 cases of nasal polyps. 2, check the project: For each income group of patients admitted to hospital are asked in detail about whether their allergies, history of asthma and respiratory symptoms, sinus coronal CT examination and conduct Lund-Mackay CT score, nasal endoscopy within Johansson endoscopic scores, allergen skin prick tests, pulmonary function tests. Pulmonary function checks the following steps: first, flow-volume curve was measured, and the results were then normal bronchial provocation test, provocation test positive on its airway hyperresponsiveness existence, diagnosis of atypical asthma; abnormal flow-volume curve then bronchial dilation test, test positive to diagnose diastolic typical asthma, dilation test negative indicates irreversible airflow limitation. Provocation test positive and flow-volume curve abnormalities which exist in some patients whose airways recessive or dominant disease, as patients with abnormal lung function. 3, according to Johansson endoscopic nasal endoscopy score 56 cases of nasal polyps in patients enrolled score was 2 or 3 points, which are divided into two groups; coronal sinus CT examination according to Lund-Mackav CT score results into group of patients with nasal polyps is divided into 0 -8 points, 9 points -16 points, 17 points -24 points of three groups; peptides according to allergen skin prick test positive or negative results into the group of patients with nasal polyps will be divided into two groups. Comparison between groups of airway hyperresponsiveness incidence of asthma incidence and the differences between lung function abnormalities. 4, statistical analysis methods: Application SPSS16.0 software for data processing, all the variables are as x ± s. Variables were compared between groups using analysis of variance, using the x2 test, P lt; 0.05 indicates statistical significance. Results: 1 in 56 cases of nasal polyps, the flow-volume curve of normal in 44 cases, including bronchial provocation test was positive in 19 cases, accounting for 33.93% of patients enrolled. Flow-volume curve abnormalities in 12 cases, including bronchial dilation test was positive in 7 cases, bronchial dilation test was negative in 5 cases, it can be diagnosed as atypical or typical asthma, 26 patients (197), accounting for all enrolled patients 46.43 %. There were a total of 31 cases of pulmonary function abnormalities (1975), accounting for 55.36% of patients enrolled. 2, in the 56 cases of nasal polyps, the detailed history of lower respiratory tract symptoms without 44 cases. In 44 patients, the flow-volume curve of normal in 41 cases, including bronchial provocation test was positive in 19 cases, accounting for lower respiratory tract symptoms in patients with no 43.18%. Flow-volume curve abnormalities in 3 patients, bronchial dilation test were negative, so the 44 patients can be diagnosed 19 cases of atypical asthma, lower respiratory symptoms without accounting for 43.18% of patients. There were a total of 22 cases of pulmonary function abnormalities (193), accounting for lower respiratory tract symptoms in patients with no 50.00%. 3, in the 56 cases of patients with nasal polyps, nasal polyps in patients with a score of 2 points with 37 cases. In 37 patients, the flow-volume curve of normal in 29 cases, including bronchial provocation test was positive in eight cases, accounting for 37 patients, 21.62%; flow-volume curve abnormalities in 8 cases, including bronchial dilation test was positive in 6 patients, bronchial Diastolic test was negative in 2 cases, the diagnosis of atypical or typical asthma by 14 cases (86), accounting for 37 patients, 37.84%; there were a total of 16 cases of lung function abnormalities (862), accounting for 37 patients 43.24%. Nasal polyp score of 3 patients with 19 cases, flow-volume curve of normal in 15 cases, including bronchial provocation test was positive in 11 cases, accounting for 19 patients, 57.89%; flow-volume curve abnormalities in 4 cases, including bronchodilation test positive 1 case, bronchodilation test negative three cases, the diagnosis of atypical or typical asthma in 12 cases (111), accounting for 19 patients, 63.16%; presence of abnormal pulmonary function in 15 patients (1113), accounting for 19 patients 78.95%. The percentage of the indicators rated 3 groups of patients treated group were significantly higher score two patients, the difference between the two groups was statistically significant, (p lt; 0.05). It showed Johansson nasal endoscopy score the higher the score, patients with airway hyperresponsiveness, with asthma and lung function abnormalities greater the likelihood. 4, in the 56 cases of patients with nasal polyps, nasal polyps Lund-Mackay CT score is 0-8 points 10 patients. In 10 patients, the flow-volume curve of normal in 9 cases, including bronchial provocation test was positive in one case, accounting for 10% of 10 patients; flow-volume curve abnormalities one case, the bronchodilation test is positive, so the diagnosis of SARS type or typical asthma, abnormal lung function in the two cases, accounting for 20% of 10 patients. Score is 9-16 points were 23 cases, 16 cases of normal flow-volume curve, in which bronchial provocation test was positive in 6 cases, accounting for 26.09% of 23 patients; flow-volume curve abnormalities in 7 cases, including bronchodilation test positive 4 cases, bronchial dilation test negative three cases, the diagnosis of atypical or typical asthma in 10 cases (64), accounting for 43.48% of 23 patients; there were a total of 13 cases of lung function abnormalities (643), accounting for 23 cases 56.52% of patients. Score is 17-24 points 23 patients, normal flow-volume curves were 19 cases, including bronchial provocation test was positive in 12 cases, accounting for 52.17% of 23 patients; flow-volume curve abnormalities in four cases in which positive bronchodilation test two cases, bronchial dilation test was negative in 2 cases, the diagnosis of atypical or typical asthma in 14 cases (122), accounting for 60.87% of 23 patients; there were a total of 16 cases of pulmonary dysfunction (1222), accounting for 23 patients 69.57%. It showed patients with nasal polyps Lund-Mackay CT score score, the greater the airway appear airway hyperresponsiveness, asthma and lung function abnormalities combined ratio the greater the difference between the groups was statistically significant (p lt; 0.05, p lt; 0.01). 5, in the 56 cases of nasal polyps in patients with allergen skin prick test was negative in 36 cases, accounting for 64.29% of all patients. 36 cases negative patients with normal flow-volume curve 30 cases, including bronchial provocation test was positive in 11 cases, accounting for 36 cases of patients with 30.56%; flow-volume curve abnormalities in 6 cases, including bronchial dilation test was positive in 4 cases, bronchial dilation test negative in two cases, the diagnosis of atypical or typical asthma in 15 cases (114), accounting for 36 patients, 41.67%; there were a total of 17 cases of lung function abnormalities (1142), accounting for 36 patients, 47.22%. Allergen skin prick test was positive in 20 cases, accounting for 35.71% of all patients. 20 cases of patients with positive flow-volume curve normal in 14 cases, including bronchial provocation test was positive in eight cases, accounting for 40% of 20 patients; flow-volume curve abnormalities in 6 cases, including bronchial dilation test was positive in 3 cases, bronchial dilation test negative in three cases, the diagnosis of atypical or typical asthma in 11 cases (83), accounting for 55% of 20 patients; there were a total of 14 cases of pulmonary dysfunction (833), accounting for 70% of 20 patients. Allergen skin prick test positive patients airway hyperreactivity, asthma and lung function abnormalities in patients were significantly higher than the negative, the difference between the two groups was statistically significant, (p lt; 0.05). Conclusion: The group of patients with nasal polyps occurred airway hyperresponsiveness, atypical or typical asthma, lung function abnormalities ratios were 33.93%, 46.43%, 55.36%, and the severity of nasal polyps and airway hyperresponsiveness, asthma, the incidence of abnormal lung function was positively correlated. So for patients with nasal polyps attention to their lung function and lower respiratory tract disease is very necessary. No lower respiratory tract symptoms in patients with nasal polyps, 50% combined abnormal pulmonary function, flow-volume curve is also 43.18 percent of normal existence airway hyperresponsiveness, suggesting that clinicians should be given adequate attention. Also in this group of patients with nasal polyps in patients with 35.71% allergen skin prick test was positive, and the airway hyperresponsiveness, asthma, pulmonary function abnormalities were higher than allergen skin prick test was negative nasal polyps. It is suggested that the lower respiratory tract disease and allergy and nasal polyps is closely related to the occurrence and severity.
