Abstract


Objective:To realize the prevalence of primary dysmenorrheal(PD) and evaluate its effect on health-related quality of life(HRQoL) among female college students, and analyze the related factors of PD, and discuss the risk factors of PD in order to provide the basis of health education and health promotion for female college students.Methods:Refering to the method in proportion, at random, stratified and cluster sampling among 900 female students of Shaoyang medical college and Shaoyang University that were carrying into a survey of self-made questionnaire, and Symptom Checklist 90(SCL-90)and Short Form-36 (SF-36). Then imput these informations into Excel2003 and analysed by SPSS 16.0. Chi-square test, Student’s t test, and logistic regression and variance analyses (ANOVA)were used for statistical analyses. On the basis of disease history, clinical appearance and gynecologic examination, bringing into 873 typical objectives including dysmenorrhea group and normal group. Then Case-control study on two groups to analyze the related factors of PD.Results:(1)The prevalence rate of PD is 61.3% (535/873), It was mild in 71.8% (384/535), moderate in 21.1% (113/535) and severe in 7.1% (38/535) of these students. (2) The single factor analysis of primary dysmenorrhea:after the comparative analysis found that thirteen factors including growth environment, Body mass index (BMI),menarche age,mother whether with dysmenorrhea history, the habits of regular meals, eating breakfast, junking food, sleep, usually sports exercise, washing with cold water in menstrual period interpersonal relationships, the pressure of study and recent anxiety in the dysmenorrhea group and the normal group were statistical significant(P<0.01; P<0.05).(3)The multiple factors analysis about PD: we put thirteen single factors that mentioned before were statistical significant for PD(P<0.01; P<0.05) by multiple stepwise logistic regression analysis.we found out that the seven factors including growth environment, BMI, menarche age, Mother whether with dysmenorrhea history, regular meals,washing with cold water in menstrual period, interpersonal relationships are affected factors for primary dysmenorrhea(P<0.05). (4)The Psychological evaluation about the two group:the difference between the five factors of Body change symptoms, Obsessive-compulsion symptoms, depression,anxiety and interpersonal sensitivity were statistical significant(P<0.05). (5) The evaluation of Female college students’ healthy related the quality of life (HRQoL): The Short Form-36 (SF-36) form was used to determinate HRQoL. we found out that the scores received many of the SF-36 domains (physical functioning, role-physical, bodily pain, general health perception, vitality and role-emotional)were lower in dysmenorrhea group than normal group. In addition, with the increasing severity of dysmenorrhea, the average scores received from all the domains of SF-36 showed decrease.Conclusion:There is a higher prevalence rate of primary dysmenorrhea in female college students(61.3%), and seriously affect on the quality of life of female college students. Heredity(Mother with dysmenorrhea history), growth environment, lower menarche age, lower BMI, the bad living hibats (non-regular meals, washing with cold water in menstrual period), negative social psychological environment are Major risk factors for PD.
