Abstract


Objective To measure the level of comfort of maintenance hemodialysis patients and explore the influencing factors of maintenance hemodialysis patients’ comfort.To evaluate the effects of auricular point magnetic bead plaster therapy on improving sleep disorders and promoting the level of comfort of maintenance hemodialysis patients.Methods1.This study developed maintenance hemodialysis patients comfort scale based on Kolcaba’s General Comfort Questionnaire(GCQ) and evaluated its’ reliability,validity and feasibility by consulting eight related experts and interviewing 100 maintenance hemodialysis patients.2.A convenient sample of 95 maintenance hemodialysis patients in a provincial general hospital provided data.The patients were interviewed and administered a questionnaire that was composed of general information, maintenance hemodialysis patients comfort scale and related factors with comfort.3.60 samples were divided randomly into two groups,the experimental group and the control group.The experimental group received auricular point magnetic bead plaster therapy and the control group received estazolam.Evaluating the effect of auricular point magnetic bead plaster therapy and estazolam with the differences of the incidence rate of sleep disorders by PSQI,the differences of the total curative effective rate of sleep disorders and the difference of the score by maintenance hemodialysis patients comfort scale between the two groups.Results1.The internal consistency(Cronbach’s alpha)for the overall scale is 0.935,and each dimension of the scale is 0.879～0.930.The retest correlation coefficient of the overall score is 0.944,and each dimension of the scale is 0.817～0.924.Four factors are extracted through factor analysis and they can explain 68.758% of the overall covariance.The factors and their subordinate items are consistency with the original hypothesis.2.The mean score of holistic comfort of the 95 maintenance hemodialysis patients is 72.91±7.60.The mean score of each item of holistic comfort,physical comfort,psychological comfort,social comfort and environmental comfort are 2.60±0.27,2.27±0.46,2.51±0.35,2.77±0.44 and 3.20±0.37 respectively. 3.The holistic comfort of the patients is correlated with age,domestic economic level,medical care system,dialysis frequency,dialysis age and the level of serum creatinine.4.Treatment fear,sleep disorders,tiredness,economy fear,lack of knowledge of disease,prolonged bed rest,dry mouth and thirst are the factors related to comfort.5.The differences of the incidence rate of sleep disorders,the total curative effective rate of sleep disorders and the score of comfort between the experimental group and the control group have statistical significance after auricular point magnetic bead plaster therapy and estazolam intervention.Conclusion1.The maintenance hemodialysis patients comfort scale has good reliability,validity and feasibility.It can be applied to nursing assessment and intervention evaluation on maintenance hemodialysis patients.2.The level of psychological comfort of the 95 maintenance hemodialysis patients is lowest,the physical comfort and the social comfort are the next,and the environmental comfort is the highest.Nurses should improve physical discomfort,focus on psychological care and promote social support.3.Nurses should pay more attention to such patients as younger,at bad domestic economic status,at themselves’ expenses,less dialysis frequency,shorter dialysis age and worse renal function.4.Nurses should take measures to intervene the influencing factors of comfort such as treatment fear,sleep disorders,tiredness,economy fear,lack of knowledge of disease,prolonged bed rest,dry mouth and thirst.5.Auricular point magnetic bead plaster therapy is good for improving sleep disorders and promoting the level of comfort of maintenance hemodialysis patients,which is safe,convenient,cheap,good acceptance and so on.
