An Investigation of Childhood Obesity in Shantou and Comparison of Screening Criteria in Childhood Obesity

Abstract


ObjectObesity is now a global epidemic. Information in Shantou children remains scanty. There are ongoing debates on the most ’appropriate’ definition and optimal cutoff values with different diagnostic criteria adopted by different cities in China. In this study, we assessed the current prevalence and its risk factors of childhood overweight and obesity in a sample of children from Shantou city. Find the differences between several various criteria.MethodsThis is a cross-sectional study. 5738 children aged 7-18 years old and their parents were randomly selected from Shantou. Anthropometric measurements including height, weight, waist circumference (WC), hip circumference (HC) were measured. BMI (body mass Index), WHR (waist to hip ratio) and WHtR (weight to height ratio) were calculated. According to WGOC criterion (as the gold standard), determine the rates of overweight and obesity of the sample categorized by age and gender. Compare with the rates according to 4 different criteria [the international obesity task force (IOTF) criterion, Centers for Disease Control and Prevention (CDC) criterion, World Health Organization (WHO) criterion and WC cut-point]. Self-made questionnaire was conducted on 414 children aged 10-14 years old and their parents. Use SPSS13.0 and Microsoft Excel 2003 to analyze the data.Results5648 children consented and were enrolled into the study. BMI and WC mean of the sample was close to rural populations in southern China. According to WGOC criterion, the rates of overweight and obesity were 6.43% and 2.82%. The rate according to WHO criterion was highest(P<0.01), with the best sensitivity and good specificity, while the rate according to IOTF criterion was lowest(P>0.05). The rate according to WGOC criterion was moderate. WC cut-point has poor compliance with other standards. The prevalence of overweight and obesity were higher in boys than in girls(P<0.01), while higher in younger than elders(P<0.01). The anthropometric measurement of WC, WHR, WHtR of obese group were significantly higher than that of non-obese group(P<0.01). The family history of obesity, parental education, late supper, food choosy, time spent on exercise were shown to be related with obesity.ConclusionsWGOC criterion is applicable for screening overweight and obesity of children and adolescents in Shantou, and rates were 6.43% and 2.82%. The obesity can be effectively prevented if children develop good eating habits, exercise habits and lifestyle. Family and schools are in a unique position for childhood health in reducing the incidence of overweight and obesity. WGOC criterion and WHO criterion were more appropriate to screening childhood obesity than other criteria.
