Abstract

Background of atherosclerosis (atherosclerosis, AS) is a major pathological basis lead to coronary heart disease. Inflammation plays a key role in the incidence of atherosclerosis, the development process. Inflammation induced acute coronary syndrome events initiating link atherosclerotic vulnerable plaque rupture, plaques inflammatory factor mRNA and protein expression was significantly increased inflammatory and anti-inflammatory cytokines imbalance plaque rupture of an important molecular biological mechanism, so the level of inflammatory factors as the evaluation of the stability of the plaque, and also as an important risk factor for coronary heart disease and predictors. Chinese anti-atherosclerosis effect has multiple targets, the advantages of the multi-link, in recent years, a variety of traditional Chinese medicine has been used for the prevention and treatment of atherosclerosis and related diseases, and shows a good role. This study investigated rhubarb inhibition of the inflammatory response of coronary heart disease, atherosclerosis, inflammatory processes and TCM Ecchymosis relationship. Objective To observe the intervention effects and clinical efficacy of rhubarb on inflammatory factors in the blood of patients with coronary heart disease, to explore the possible mechanism of its inhibition of the inflammatory response of coronary heart disease. Method 1 object selected object case group for June 2009 to 2010 in March Qilu Hospital of Shandong University, Department of Cardiology, admitted 50 cases of patients with coronary heart disease, were randomly divided into the Chinese intervention group and the conventional treatment group. Another 20 cases of physical examination no positive signs of healthy subjects were selected as a control group. Questionnaire in the form of records of all patients with clinical information, including age, sex, smoking history, blood pressure levels, lipid levels, blood glucose, family history of cardiovascular disease and other projects. 10 weeks of treatment, all patients were followed up once every two weeks. 2 lipid testing serum total cholesterol (total cholesterol, TC), triglycerides (triglyeride, TG), low-density lipoprotein cholesterol (low-density lipoprotein cholesterol, LDL-C), high-density lipoprotein cholesterol (all patients before and after treatment the level of high-density lipoprotein cholesterol, HDL-C). 3 TLR-4 detected in all patients before and after treatment, early morning fasting venous blood. By density gradient centrifugation and magnetic bead selection and separation of peripheral blood CD14 monocytes and CD14 monocytes Toll-like receptor 4 (Toll-like receptor-4 was detected by flow cytometry, TLR-4) expression TLR4 mRNA expression levels, real-time quantitative RT-PCR assay. Inflammatory cytokines were detected by ELISA (enzyme-linked immunosorbent assay, ELISA) detection of serum interleukin -6 (interleukin-6, IL-6), tumor necrosis factor-a (tumor necrosis factor-a, The expression levels of TNF-a) and high-sensitivity C-reactive protein (high sensitivity C-reactive protein, hs-CRP). 5 patients enrolled in clinical efficacy evaluation after treatment of angina pectoris, ECG efficacy evaluation. SPSS16.0 statistical software of 6 statistical analysis applications processing, measurement data were expressed as x ± s, paired t test was used to compare the group, between the two groups with a group t test used to compare rates between groups χ2 test. P lt; 0.05 for the difference was statistically significant. Results Two patients failed to complete all the experiments, with a total of 48 cases of patients with follow-up data of a complete blood which the Chinese intervention group (26 cases), the conventional treatment group (22 cases). A lipid testing: Chinese intervention group after treatment, TC, TG, LDL-C decreased significantly (P lt; 0.01), compared with the conventional treatment group compared with that before treatment, the the Chinese intervention group TC, TG was significantly lower (P lt ; 0.01), LDL-C level decreased, but did not reach statistical significance. Was no significant difference between the two groups of HDL-C levels. 2 TLR4 detection: (1) compared with the healthy control group, the average TLR4 Mean values ??in the the the Chinese intervention group and the conventional treatment group and the mRNA levels significantly increased (P lt; 0.01), while the the Chinese intervention group and the conventional treatment group between no significant difference (P gt; 0.05). (2) Compared with before treatment, TLR4 Mean values ??and mRNA expression levels in the the Chinese intervention group after treatment was significantly lower (P lt; 0.01); comparison with the conventional treatment group, the Chinese intervention group of TLR4 Mean values ??and mRNA expression levels significantly decreased (P lt; 0.01). 3 serum inflammatory factor test: (1) compared with the healthy control group, the the the Chinese intervention group and the conventional treatment group before serum TNF-a, IL-6 and hs-CRP levels were significantly raised (P lt; 0.01) no significant difference between the the Chinese intervention group and the conventional treatment group (P gt; 0.05). (2) Before treatment comparison, traditional Chinese medicine intervention group and the conventional treatment group after treatment serum of TNF-a, of IL-6, hs-CRP's expression of water on average significantly with decreased (P lt; 0.01); compared with the conventional treatment group after treatment, The Chinese intervention group indicators significantly lower (P lt; 0.01). 4 clinical efficacy: two groups of patients after the clinical efficacy of the total efficiency between the significant difference (P lt; 0.01), the Chinese intervention group was superior to the conventional treatment group. Conclusions The expression of inflammatory cytokines in the blood of patients with coronary heart disease was significantly elevated, illustrate the importance of anti-inflammatory therapy in coronary atherosclerotic heart disease prevention. 2. Rhubarb through the regulation of blood lipids and inhibit the expression of TLR4 and inflammatory factors, and thus play a role in anti-atherosclerosis. 3 anti-inflammatory effects of Chinese medicine, Western medicine treatment of coronary artery disease, the clinical efficacy superior to conventional western medicine treatment.
