Abstract


Objective:Discussion of traditional Miao drug fumigation therapy on experimental knee osteoarthritis in rabbits with early effects and mechanism of chondrocyt apoptosis and proliferation.Methods:Model of articular cavity injection of papain in manufacture of KOA, Miao drug fumigation treatment after20days, naked eye, under optical microscope observation on pathological changes of cartilage tissue, Mankin, score by; NO nitrite reductase assay synovial fluid level; Flow cytometric detection of chondrocyte apoptosis index (AI); Immunohistochemical detection of articular cartilage by Bax, Bcl-2level.Results:(1) visual observation model group can see the change of the articular cartilage surface;(2) in the light microscope can be observed: the normal group:4layers of the articular cartilage structure is clear, smooth cartilage integrity, cartilage cells arranged in neat rows, columnar layer chondrocytes were columnar, below the tide line, the middle layer of cartilage cells was disorderly, scattered in the arrangement, the phenotype of a circle. Matrix staining is uniform, the surface of the cartilage cells is approximately horizontal arrangement of spindle-shaped phenotype. Model group:severe structural damage of articular cartilage layer, the surface is not smooth, a grid-like hardening, some surface absence of cartilage almost completely disappeared, instead of the proliferation of fibrous connective tissue, synovial part of the nodular hyperplasia, cartilage cells decreased, exposed subchondral bone, osteophyte formation, angiogenesis, incomplete or disappear tide line. Fumigation Group:articular cartilage layer hierarchy of legible, significantly reduced cartilage destruction, cartilage surface than the rule, smooth and complete, the cell arrangement rules, clear tide line. Surface cells arranged in mild disorder or see mild deformation did not change in the middle and deep, subchondral bone and trabecular bone density did not change significantly. Water group:4-layer structure level of the articular cartilage can still be identified, cartilage surface roughness, a little uneven, the deletion of part of the surface cells, no middle and deep change, the cartilage cells showed diffuse hyperplasia, the tide line blurred, or even to see the double tide line;(3) of each group mean Mankin’s’s score display:Compared with normal group, model group the mean Mankin’s’s score increased a, with very significant sex difference (P<0.01), compared with model group, the treatment groups in mean Mankin’s’s ratings are lower the (P<0.05), Miao medicine fumigation group has a very significant difference (P<0.01), Miao medicine fumigation group and water group, the difference was not statistically significant (P>0.05);(4) Annexin v/PI double parameters to test results showed:normal chondrocytes apoptosis, compared with model, significantly lower than model group (P<0.01), Miao drug fumigation group compared with model were significantly different (P<0.05). The water group compared with model, no significant difference (P>0.05);(5) seedlings drug fumigation KOA model of rabbit synovial fluid of NO results:NO concentration of rabbit synovial fluid of the model group than in normal group significant. Compared with normal group was statistically significant (P<0.01), the synovial fluid of the seedlings drug fumigation group NO concentration decreased, compared with model group, the difference was statistically significant (P<0.01), Miao medicine fumigation group and water group, the difference was statistically significant (P<0.05);(6) normal group, a small amount of expression of Bcl-2positive cells; model and treated groups were significantly, with the normal group there was a significant difference (p<0.01between); expression rate in each treatment group compared with the model group increased (p<0.05), of which the fumigation group has a very significant difference (p<0.01), Miao drug fumigation group and the water control group, a significant difference (p<0.05);(7) of Bax in the normal group, a small amount of expression, modeling and treatment significantly increased compared with normal group, model group, seedlings drug fumigation group and water in the control group showed significant differences (p<0.01), compared with model group, the treatment groups were significantly different (p<0.05), Miao drug fumigation group and the water control group, significant difference (p <0.05).Conclusion:Miao drug fumigation therapy can inhibit apoptosis of osteoarthritis of the knee cartilage, improve cartilage lesions; May inhibit cartilage inflammation, which means NO release reduction, reduction of chondrocyte apoptosis index, Bcl-2, inhibiting the expression of Bax level related to the expression of. Modern pharmacological studies have confirmed that the experimental seedlings drug prescription have better analgesic, anti-inflammatory and antibacterial effects, which may be one of the mechanisms Miao medicine the Fumigation improve cartilage inflammation. And variety of drugs associated with microcirculation, these effects may be through inhibition of inflammatory factor release, eliminating free radical damage, improve the microcirculation in the bone to promote cartilage matrix synthesis’ to inhibit chondrocyte apoptosis, improving cartilage lesions.
