Abstract

Delirium is a transient confusional state caused by several factors; aclincal neuropsychiatric syndrome with core features of acute onset; themost common behavioral manifestation of acute brain disfunction. OnceDelirium occurs, the hospitalization time will be obviously prolonged, andthe reintubation rate, death rate and hospitalization cost per day will beincreased. Domestic and international research of recent years pay muchattention to delirium and obtain certain effects. The research of domesticnursing staff over the pas decade is mainly dedicated to the risk factors andintervention measures of ICU delirium patients, not aware of theimportance of the knowledge and the assessment ability masterd by theICU nurses in early prevention and assessment.This research surveyed the status of knowledge, attitude and practiceof ICU nurses of third-grade general hospitals. In order to improve theability of DICU nursing, we also offered the feasible and superiorsuggestion and put forward the key emphasis in practice and education, Allthese supplied the theoretical basis for the behavior intervention of the next step.Objectives： To evaluate current perceptions, attitudes and practicesof nurses in ICU regarding delirium in the ICU (DICU) comprehensively.Methods： A self-administered questionnaires regarding DICU wasdistributed to384ICU nurses in five first-level hospitals in Chongqing.Results：A total of384valid questionnaires were collected.“Totallyclear” was subjectively selected by9.1%of the respondents with regard tocomprehensive knowledge of DICU, while32.8%of them selected correctoptions less than50%and only7.5%more than75%in the objective test.College education was considered to be their main source of the knowledgeabout DICU in53.2%of the nurses. While，only8.9%of the respondentsreceived further training and7.3%of them self-study commonly for thisdisease in the work. Definitely, college education was not believed to beenough for the clinical practices by all of the nurses. Meanwhile, the vastmajority of the nurses admitted that they should participate the managing ofDICU （86.0%） but their knowledge and only6.5%of them wereconfident for this work. In clinic, about a quarter （18.6%） of the nursesevaluated the psychological situation of patients in the ICU and specificcare records for patients’ psychological situation was applied by15.8%ofthem routinely.Conclusions: The vast majority of ICU nurses in Chongqing lackeither knowledge to serve the clinical practice or further training on, especially the monitoring and managing of, DICU. They have rare practicesand positivity to self-study on this focus although the importance of theseknowledge and abilities was admitted by them. Accordingly, systemicfurther education on DICU should provided for the ICU nurses andprotocols for the screening and monitoring of DICU should be introducedin nurses’ clinic practice to improve the security of ICU patients’management，resulting cost of remediation，improving the safety of patientsand increasing the reliabilityof patients in health care activities．
