Objective: Observation in comparing different fixed method to treat distal humeralfractures, the clinical curative effect for clinical surgical treatment of distal humeralfractures provide reference.Method:A retrospective analysis was conducted of theeligible59patients who had undergone surgical treatment of distal humerus fractures inour hospital between January2008to January2012. They were34men and25women,18to65years of age (average,40years). According to the AO/ASIF classification,14caseswere of type A,18cases of type B,27cases of type C. Treatment using three differentfixed methods of distal humerus fracture.20patients were managed by Single plate (groupA),17patients were managed by Y type plate (group B),22patients were managed bydouble plates (group C). The material of the surgical approach, inner fixed mode andpostoperative recovery elbow function analysis. Result: The59patients were followed upfor an average22months(from12to36months),fracture general11～17weeks to heal,average healing time of14weeks,all cases occurred postoperatively incision skin necrosisand deep infection,1cases of ulnar nerve paralysis,1cases with myositis ossificans.Double plates fixed no looseness and fracture, Single plate fixed appear in1loose and1cases of fracture, Y type steel plate fixed appear in1sheet loose situation. By the Myaoelbow performance score,35cases were rated as excellent；12cases as good;9cases as fairand3cases as poor, with an excellent to good rate of79.7%. Conclusion: According tothe different approaches and inner fixed method curative effect analysis, we think ulnaolecranon cut bone into the way can make the elbow joint articular surface fully revealed,and at the same time avoid the tricep damage, protect the antecubital soft tissue, brachialthree head tendons and collateral ligament, muscle healing into osseous healing, behelpful for early functional exercise, especially for the C3type fracture treatment, obviousadvantages.Double plates fixed, conform to the distal humerus biomechanicscharacteristics, can provide the lasting will strong fixed, is advantageous in the elbow joint function recovery. For patients with distal humerus fractures, the treatment should bebased on patients condition and fracture type choosing appropriate surgical approach andinner fixed method, early postoperative function exercise, and avoid complications.
