Abstract

Objective and background Endometrial polyps is (Endometrial polyps, EPs)limitations of Endometrial hyperplasia, can occur in any position of uterine cavity or thecervix, is a common frequently-occurring disease of department of gynaecology, butbecause most patients with endometrial polyps often without any symptoms, and somecan natural fade as the menstrual cycle, therefore, the exact prevalence is unknown.EPmorbidity age multiple women at the age of35,35women the incidence of around23%,while the women under35of EP probability is about3%.In spite of endometrial polyps is the clinical common disease, but its etiology andpathogenesis is not clear. There are most of the research found that the disease may beassociated with endometrial local female progestational hormone and its receptor whichis unbalanced. When local endometrial estrogen and its receptor expression is increased, but the antagonism of progestin and its receptor expression is reduced, which leads tothe department of endometrial proliferation state for a long time and lost the function ofthe normal endometrium,so polyp may eventually formed. The molecular basis ofendometrial cell hyperplasia is the information of extracellular molecules estrogen。when they combine with estrogen receptors (ER),estrogen receptors are activated。AndER not only regulate the expression of genes, but also can adjust the promotersequences of gene expression that are rich in GC.17beta-hydroxysteroid dehydrogenase1(17β-HSD1) is a member of17beta-hydroxysteroid dehydrogenase family(17β-HSDs).17β-HSD1can catalyze estrone and androstenedione which are lowbiological activity to estradiol and testosterone which are high biological activity。And the promoter sequences of the genes that code17β-HSD1are rich in GC.Therefore thepurpose of this experiment is to study the expression and significance of ER and17β-HSD1in endometrial polyps,in order to provide strong theoretical basis for thediagnosis, treatment, prevention and relapse of endometrial polyps. Methods Immunohistochemical method (sp) were used to detecte the expression of17β-HSD1and ER in endometrial polyps, and the cells are positive cells whose nucleusor cytoplasm appear yellow or tan granule. Results In polyps group, the positive rate of17β-HSD1expression was77.2%,in normalendometrial tissue in the same patient uterine cavity where is far away from endometrialpolyps, the positive rate of expression of17β-HSD1was20%(χ2=20.42,P=0.000),,and in normalendometrial tissue the positive rate was25%(χ2=17.085,P=0.000).The difference is statisticallysignificance.So17β-HSD1expression in endometrial polyps is also significantly higher than thecontrol group, while there is no difference between control groups(χ2=0.22,P>0.05)。In polyps group, the positive rate of ER expression was81.7%,in normal endometrialtissue in the same patient uterine cavity where is far away from endometrial polyps, thepositive rate of expression of ER was16.7%(χ2=22.823,P=0.000),and in normalendometrial tissue the positive rate was21.7%(χ2=21.624,P=0.000). The difference isstatistically significance. That illustrate that ER expression in endometrial polyps issignificantly higher than that of the control group, while there is little difference betweencontrol group1and group2in the expression of ER(χ2=0.006,P>0.05).The patients with endometrial polyps were further be grouped and analysed.Wefound that the positive rate of ER expression in postmenopausal endometrial polyps(88%)is slightly higher than that of premenopausal patients with endometrial polyps(77.4%). As opposed to ER,17β-HSD1expression positive rate in postmenopausalendometrial polyps (76%) is slightly lower than premenopausal patients withendometrial polyps (80%).But after statistical analysis, the difference of ER and 17β-HSD1expression in endometrial polyps beteeen premenopausal andpostmenopausal patients is not statistically significant (P>0.05).Conclusions Excessive expression of17β-HSD1and ER in endometrium may be thereason of occurrence and development of endometrial polyps.
