Clinical effect of modified method for cesarean section in a pregnant woman

Abstract


Objective:By comparing the differences between the modified cesarean section and classical lower uterine segment crosscut mouth cesarean section, to explore the advantages and disadvantages of the modified cesarean section, and to provide scientific basis for Further Application of it.Method:Using the menthod of historical control study, random selection one hospital, in strict accordance with the criteria and exclusion standard, the selection of the traditional lower uterine segment cesarean section was150mothers as the control group from jan2010to jun2010hospital of cesarean, using modified new cesarean section was150mothers as the treatment group from jan2011to jun2011hospital of cesarean. Using t test or Chi-square test, comparing the difference of the nine indexes:total operation duration time, fetal childbirth time, postoperative incision pain time, postoperative incision exhaust time, before open amount of blood loss, fascia hematoma, puerperal morbidity, fat liquefaction rate and Apgar score of neonates between the control group and the treatment group.Result:①The general demographic characteristics of the treatment group are24.96years old of the average age,3.58times of pregnant times,2.81times of production times including1.23times of the numbers of cesarean delivery times and39.28weeks of gestational week, and the control group are24.55years old,3.33times,2.57times including1.19times of the numbers of cesarean delivery times of and39.12weeks respectively. There was no significant difference of these indexs between the two groups (p>0.05).②Cesarean section first indications:Fetal distress in cesarean section first indications of two groups was most, the next was cicatricial uterus, placental abruption, omphaloproptosis and pregnancy associated with cardiac disease were least. The differences of cesarean section first indications between two groups were not statistical significance(p>0.05).③The total operative time (average18.14scores)and the fetal childbirth time(average2.68scores) of the treatment group were lower than those(average40.07scores and8.28scores) of the control group, and the differences had statistical significance(t=50.549and47.167,p<0.05)④The postoperative incision pain time (average10.92hours) and the postoperative incision exhaust time (average10.13hours) of the treatment group were shorter than those(average23.31hours and27.88hours) of the control group, and the differences had statistical significance(t=30.166and33.236,p<0.05⑤The amount of bleeding loss before open surgery of the treatment group(average1.49pieces of gauze) was less than that of the control group(average4.04pieces of gauze). And there was statistical difference between the two groups (t=24.416, p<0.05).⑥The incidence of fascia hematoma after surgery (0%) and puerperalism (7/150=4.7%) of the treatment group were lower than these(6/150=4.0%and20/150=13.33%) of the control group. There were statistical differences between the two groups (χ2=4.252,p<0.05). But there was no significant difference of fat liquefaction in surgical incision (0.0%and3/150=2.0%) between the two groups (χ2=1.347, p>0.05).⑦The Apgar score of neonates respectively was8.97on average and7.90on average in the treatment group and the control group. There was statistical differences between the two groups (t=-8.368, p<0.05).Conclusion:The results indicated that there was significant decrease in total operation time, fetal childbirth time, postoperative incision pain time, postoperative incision exhaust time, amount of blood loss before open surgery, fascia hematoma and puerperal morbidity of modified cesarean section compared with classical lower uterine segment crosscut mouth cesarean section in the average ages, pregnant times, production times and the numbers of cesarean delivery times no obvious difference between in the treatment group and the control group,and the Apgar score of neonates was be increased. Thus this suggests that modified cesarean section has a certain value in clinical popularization.
