Clinical Features and Prognostic Analysis of the Primary Lung Cancer Patients Experience at Fujian from2009-2012

Objective Primary lung cancer is one of the most common malignant tumors.Toimprove the current understanding the naturel history and influence factors,we need tostudy the clinical features and the corresponding prognostic factors.Methods we collected date of patients of permanent population in Fujian province whodiagnosed primary lung cancer from January,2009to Aguest,2012in Fujian provincehospital.Gender,age,smoking history,pathology,TNM stage,condition of nutritionand the choice of treatment were analyzed for their relationships to prognosis.Thesurvival rate was calculated by Kaplan-Meier method and Log-rank test was used tocompare the survival rate.Univariate and multivariate factors for survival were analyzedby Cox proportional hazards regression mode1.Results! Of the831patients,male to female rato is2.21.90.9%of all patients wereNSCLC and9.1%were SCLC.The predominant pathological type in all the patients wasadenocarcinoma(52.5%).The highest incidence of pathologic types in never-smokers wasadenocarcinoma(72.3%)while squamous carcinoma was the predominant pathologicaltype in former-smokers(37.4%) or current-smokers(45.6%).Fiberoptic bronchoscopywas the most widely-used means of diagnosis (51.1%),next was the.The overall MSTwas24.60±1.99months,and the overall1-yr,2-yr,3-yr and4-yr survival rates were72.0%,53.6%,44.1%,37.0%.The MST of NSCLC was26.30±2.05months while SCLCwas14.23±1.32months.Univariate analysis revealed that gender (P=0.001),ages(P=0.007),smoking history (P=0.023),TNMstage (P=0.000),choice of treatment(P=0.000),ALB (P=0.000),HB (P=0.000) were prognostic factors.Cox multivariateanalysis revealed that gender,TNM stage,the choice of treatment and HB wereindependent prognostic factors for NSCLC.And cox multivariate analysis also revealedthat the choice of treatment was the independent prognostic factors for SCLC.Conclusions By analyzing the clinical characteristics and the related prognostic factorsof lung cancer,we can understand the epidemiological characteristics of lung cancer in fujian in recent years and the part of the prognosis factors of lung cancer.Thus,we canguide the clinical diagnosis and set individualized treatment plans in order to improve theprognosis of patients.
