Abstract


Objective:To evaluate the significance of the embolization of renal artery beforeradical nephrectomy in patients with intermediate-and advanced-stage of renal cellcarcinoma.Methods:Seventy-three patients with intermediate-and advanced-stage o f renal cellcar cinoma were divided into two groups. Patients in treatment group (n=43) underwent radical nephrectomy3to10days after preoperative chemoembolization withlipiodol and gelfoam. Patients in control group(n=30) received only a radicalnephrectomy. Intraoperative changes in the tumor and surrounding tissue, blood loss,operating time, and postoperative survival rate were compared between the twogroups.Results:All the patients in the treatment group had a successful radical excisionsurgery1to5days after renal artery embolization therapy. Intraoperative observedthat renal artery pulsed abate even disappear.The renal vein and the tumor surfacevessels into withering and oedema renal weeks tissue which was beneficial inseparation; Post operative pathologic description: the resection kidney wasischemic changed with the central necrosis and there were large thrombosis in kidneyvessels and tumor blood. There were significant differences in blood loss, operatingtime, between treatment group and control group.(318.14±100.34mL vs536.17±128.91mL,177.56±35.68min vs199.73±25.874min, all P <0.05).Conclusion:Preoperative embolization of renal artery is beneficial to radicalnephrectomy in patients with intermediate-and advanced-stage of renal cellcarcinoma.
