Abstract


The research is to investigate the recipients for kidney transplantation by medical institutions in Guangzhou area. Random sampling method was used in the research and the samples were collected before and after transplantation six months. The questionnaires included SF-36 by Boston Health Study and Social Support Rating Scale (SSRS) by Shuiyuan Xiao. Descriptive statistics,t-test,ANOVA,multiple linear regression and liner correlation were used to analyze the recipients’social support and quality of life status and evaluate their relationship and interrelated factors. Furthermore, we tried to find out the science solutions to improve their quality of life,choose suitable treatments and provide the corresponding health economic evaluation on the basis of the research.The investigation results showed:①The total score of social support of kidney transplantation recipients was 42.31±9.65 in this research and it had significant difference comparing to the normal (34.56±3.73). There were significant difference in social support scores among recipients with different genders, age, educational background, marriage status and back to work and study or not. The males gained more social support than the females, while the females seemed to utilize such support more effectively. Furthermore, the younger recipients (<60 year-old) and the recipients with spouses and higher education levels were likely to get more social support than the elderly (≥60 year-old) and those ones with poor education background and without spouses. On the other hand, the people back to work and study after transplantation would have more support from the society.②The differences in all scales of QOL of kidney transplantation recipients after transplantation 6 months, including Physical Functioning(PF) , Role Physical(RP),Bodily Pain(BP) , General Health(GH) , Vitality(VT) , Role Emotional(RE) , Mental Health(MH) and Social Function(SF) , were of statistical significance comparing to the normal, especially significant in PF, MH and RE.③The total score of the quality of life(QOL) for the recipients was 592.07±92.46 after kidney transplantation 6 months, and it was higher than that of before transplantation. Their difference was of statistical significance (P<0.01).The total scores of QOL were of no significant difference in patients with different genders and age. This investigation indicated that the score of QOL had positive correlation with economic level, understanding of health knowledge after kidney transplantation and social support, and negative correlation with complication incidence.④Social support of the recipients was significant positive correlation to total score of QOL, physiological health and mental health (P<0.01).The research indicates kidney transplantation can improve QOL of chronic renal failure patients, including PF,RP,BP,GH,MH,RE,SF and VT. The factors relative to QOL in transplantation recipients are of many aspects varing with education background, marriage status, economy level, the payment of therapy, back to work or not, the resource of kidney, the knowledge of health, complication evidence, etc. Above all, their QOL largely depends on their social support. Medical staffs must give them targeted guidance and particular rehabilitation education, especially to the elderly and the patients with lower education level and poor knowledge acceptance or in poor economic conditions and family relationships. A fully following-up assignment must be arranged and a good social support system must be established to provide nursing intervention in order to improve their QOL.
