Abstract


Objective: greater trochanter bone flap combined with bone marrow mesenchymal stem cells to treat avascular necrosis of the clinical efficacy of follow-up observation and conclusion , explore greater trochanter bone flap combined with bone marrow mesenchymal stem cells to treat avascular necrosis , especially in the early or mid- early (Ficat Ⅱ - Ⅲ) clinical efficacy. For clinical conduct such operations provide a theoretical and practical base . METHODS: September 2008 to October 2010 for 22 cases ( 34 hips ) femoral head necrosis (ONFH) underwent greater trochanter bone flap combined with autologous bone marrow mesenchymal stem cell transplantation in the treatment and clinical follow-up of patients obtained in accordance with international staging criteria (Ficat or ARCO) staging classification were analyzed retrospectively ; application Harris hip score system and be changed before and after surgery Ficat staging imaging evaluation . Will be confirmed 22 cases of 34 hips were randomly divided into treatment group ( ie the application combines greater trochanter bone marrow mesenchymal stem cells ) and the control group, the control group was treated with drugs to promote local microcirculation and combined electrical stimulation and hyperbaric oxygen , and with topical medication , the treatment group and the control group in all patients with hip, knee pain , limping 18 cases , 13 cases of hip joint dysfunction . The treatment group and the control group age , Ficat staging and pathological staging causes and circumstances were statistically analyzed with balance, and showed comparable. Results: 22 patients on postoperative regularly followed up for 3 to 24 months ( mean ] 6.2 months ) , the treatment group hip pain relief 9 cases ( 82% ) , joint function improved seven cases ( 64% ) , walking prolonged 8 cases ( 73% ) . Postoperative results were satisfactory rate of 77.8% . Postoperative hip preservation was 73.0% ~ 92.2% , of which idiopathic traumatic and postoperative hip preservation rate and improved hip function better, steroid and alcohol is poor. The treatment group compared with the control group , after good rate compared with the total efficiency of a significant difference . Conclusion : Exercise greater trochanter bone flap combined with autologous bone marrow mesenchymal stem cell transplantation in the treatment method really reliable, patient compliance is better than the control group using drugs to promote local microcirculation and combined electrical stimulation and hyperbaric oxygen , and with the efficacy of topical drugs well, is a treatment for avascular necrosis , especially Ficat Ⅱ - Ⅲ of more effective treatments.
