Exploring the Related Factors of the Nutrition Status of Dementia Patient with Dyphagia

ObjectivesThe objectives of this study were to investigate the nutritional status of demented patients with dysphagia by questionnaires in four general hospitals and analyze the relevant factors of malnutrition from patient’s own condition and caregivers two aspects,so as to provide theoretical basis for the establishment of clinical nursing intervention measures.Methods106demented patients with dysphagia and their caregivers were selected by random and convenient sampling method from4general hospitals. Evaluation of swallowing function was trough Harada functional evaluation method.The questionnaire included socio-demographic characteristics, MNA-SF, EdFFD, ADL,ZBI and MMSE.After data entry SPSS17.0package,statistically descriptive and analysis were carried out.The Spearman and Pearson correlational analysis was used to analyze the correlation between variables and logistic regression analysis was used to analyze primary related factors.Path analysis of structural equation model was used to explore the relation between the nutritional status and related factors as well as the relationship between these factors.Results1.In our study,rate of effective questionnaires was90.59%.The average score of MNA-SF was7.79±2.67.The rate of malnutrition,having risk of malnutrition,normal nutrition was50.00%,38.68%and11.32%respectively.2.Univarate analysis shows that the related factors of malnutrition were age, family’smonthly income,chronic case history, masticatory function, food-intake, dysphagia,difficulty in feeding, activities of daily living, cognitive function, caregiver burden(P<0.05).Multiple Logistic regression analysis showed that chronic case history, food-intake,dysphagia,caregiver burden were the main related factors of malnutrition. If we got rid of the confound factors and add a level of dysphagia,the risk of malnutrition would increase to1.666times of the original.3.Path analysis showed that activities of daily living,caregiver burden, food-intake had direct effect on patients’nutritional status and could pay40.00%responsibility in the change of nutritional status. Furthermore,dysphagia, difficulty in feeding,cognitive function had indirect effect on patients’ nutritional status.4.The score of MNA-SF was middling correlated with traditional nutritional assessment indexes of Alb and TP.It had low correlation with PA、 GLO and Hb,the correlation coefficients were0.405、0.416、0.363、0.203、0.227respectively.Conclusion1.The prevalence of malnutrition is high in dementia patients with dysphagia. The main related factors are chronic case history, food-intake,dysphagia, caregiver burden.2.Activities of daily living,caregiver burden,food-intake has direct effect on patients’nutritional status.Dysphagia and difficulty in feeding affect patients’nutritional status through food-intake.Cognitive function affect patients’nutritional status through caregiver burden.
