Abstract

Objective:To evaluate the clinical effect of fiber post on incisor residual crowns and roots after the rehabilitation by composite resin and all ceramic crown.Methods:Doctors communicate with patients to understand the patients’demands and expectations and make a diagnosis and treatment plan combining with the medical history and clinical examination results. Patients are required to sign the consent after the prognosis and risk informed.52teeth of26patients were collected from March.2011to September.2012in prosthodontics department of the Affiliated Stomatologic Hospital of Anhui Medical University. Oral examination was carried out and preparation was made for treatment, including removing the bad restoration, completing root canal therapy, periodontical treatment and necessary surgery. Patient without any discomfort within one week and with the healthy periodontal condition shown from X-ray are ready for treatment. Glass fiber post was bonded with composite resin and then core was moulded, and healthy dentin was kept to the maximum based on the principle of post preparation. Prepare according to the requirements of tooth preparation and make the all ceramic crown restoration. Inform patients the related notes and the appointment time. Then record the clinical results and observe the clinical effects in terms of the six espects (crown loss, frature rate of post-core, root fracture, gingival inflammation, gingival border color, secondary periapical periodontitis)..Results:All the patients were recalled during the followed6to12months. Radiological method and clinical examination were applied to evaluate the repaired effect of fiber post.(1) During the restoration period,96.2%were successful. The appearance, color and retainer of the all ceramic crown were satisfactory. The restorations were margin sealed, roots were firmly intact, no root fracture, no post loose and fracture, no periodontal problem, no secondary apical paradentitis.(2) Two restorations failed. One case was the post dislodged when four months after the complete of restoration, the other case was the post fractured and gingivitis occurred seven months later.Conclusion:The placement of the fiber post creates an unnatural restored structure since it fills the root canal space with a material that unlike the pulp, has a defined stiffness to support the core material..The elastic modulus of glass fiber post (30-40Gpa) and composite resin (20Gpa) are similar as that of dentin (15-25Gpa),, The similar elastic modulus show that they fall in a biomechanical homogeneity unit, so that the stress to the root could be distributed, and mode of failure is unlikable to occur Therefore secondary treatment would be carried out. On the other hand, the fiber posts have superior property of esthetic, bonding and biocompatibility than cast metal posts. Thereby, they would be a good choice for restoration of incisor residual crowns and roots.The mechanical properties of fiber post lead to the stress concentrate on the neck of tooth which was repaired, so the ferrule effect was extremely important when used fiber post. In light of the less tissue of the tooth, enough ferrule structure in the flared root could not be obtained. Treatment should be taken after fully careful consideration and much longer clinical observation is suggested.
