Abstract


Objective:To explore the efficacy of self-locking appliance technology with non-extraction, used self-locking appliance technology with non-extraction to treat dentitionmoderate crowding with bone class I of the patient and observed changes of hard tissueand soft tissue profile before and after treatment.Methods:Selected one patient of bone class I admitted by Stomatology Hospital ofShenyang on February2010,female,26years old,UL crowding6mm,LL crowding4mm,front teeth occlusal anti-covering I,bilateral molars and canines near relations.Throughclinical examination,the analysis of the patient’s plaster model and cephalometric results,combined with requirements of patient,applied self-locking orthodontic technology withnon-extraction to lift crowded dentition.Taked advantages of self-ligating brackets,(suchas low friction to promote tooth moving rapidly, controlling the teeth at the3d precise,protecting the health of the periodontal tissues, shorten the time of treatment, etc.)to liftthe crowding of upper and lower, aligne dentition,levele arch.This case selected3MSmartclipTMself-locking bracket appliance.At the process of treatment, the patientweared the splint to remove occlusal interference of the front teeth and depress posteriorteeth to control craniofacial vertical height. Intermaxillary traction adjust dentalocclusion.Before and after treatment, taked X-ray photoes and selected E line、Downsanalysis and the Peking analysis measurement, a total of24, to observe the changes.Results: Correction course was27months. After the treatment, upper and lowerdentition arranged in neat rows, midline of dentition centered, bilateral canines、molarsas a neutral relationship,the front teeth established normal overbite coverage;temporomandibular joint opening and opening type were normal, bilateral joint did nothave bullets rang, bilateral masticatory muscle did not have tenderness,tooth occlusaldid not have interference, no other bite problems; facial cephalometric improved, patientwas satisfy with correcting results. After the correction, contrasted dental arch of upperand lower, displayed:(1)dental arch of upper,the length of preceding increased5mm,themiddle length decreasee1.5mm; dental arch of under, the length of preceding increased2.5mm,middle length decreased1mm; the length of the upper and lower dental archincreased.(2)The middle width of upper dental arch increased1mm,the preceding widthof lower dental arch increased3mm, the middle of increased4mm. Comparison of thedata showed before and after the treatment:(1)SNA increased1°,the upper jaw growedat the level of forward direction;(2)U1-SN increased8.8°,U1-NA increased6.7°,L1-MPincreased5°,L1-NB increased4.7°,U1-L1reduced10.5°,the lip inclination of the frontteeth of the upper and lower increased, the protrusion increased;(3)MP-FH increased1°,MP-SN increased0.5°,high angle increased slightly;(4)NP-FH increased1.5°,NA-PAincreased2.5°,maxillary relative to the entire side increased;Y-axis angle decreased1°,prominent chined;(5)NLA reduced7°,Ls-E increased2.2mm,Li-E increased1.5mm,ULP increasee1mm, LLP increased1.5mm, soft tissue profile improved.Conclusion:1. Using self-locking orthodontic technology with non-extraction totreat bone I dentition moderate crowded achieved good clinical efficacy;after traetment,teeth ordered, the center line center-aligned, anterior established normaloverbite,canines and molars reached neutral relationship,original facial profilesimproved, patients were satisfied with the correction results.2.SmartclipTMself-lockingbracket expanded the length and width of the arch, relieved the moderatly dentitioncrowding.
