Quantitative Analysis the Features of Benign and Malignant Gastric Mucosal Lesions: the Initial Experience of Spectral CT

Abstract


Purpose:To investigate the quantitative analysis value of spectral computed tomography(CT) in differentiating benign and malignant mucosal lesions.Materials and Methods:64patients with gastric cancer (GC),48patients with gastricinflammation (GI) and50patients with normal gastric mucosa (NG) were retrospectivelystudied in cohort. All the patients underwent double phase (arterial phase, AP and portalvenous phase, PVP) enhancement spectral CT performed. The mean Iodine concentrations(ICs, mg/ml) of GC, GI, and NG were calculated. An independent sample t test wasperformed to compare mean ICs between these groups. Diagnostic performance wasevaluated by receiver operating characteristic (ROC) curve analysis.Results:The mean ICs of GC (24.19±8.27, AP;28.00±7.01, PVP) was significantlyhigher than those in GI (19.07±5.82, AP;24.66±6.55, PVP) both in AP and PVP(P=0.00,P=0.00). With the use of the optimal cut-off value of AP20.00and PVP25.75, asensitivity and specificity of43%and85%were yielded, and the area of ROC were0.70and0.64in AP and PVP respectively. The mean ICs of GC(24.19±8.27, AP;28.00±7.01,PVP) was significantly higher(P=0.00,P=0.00)than those in NG (12.71±2.13, AP;15.80±2.87, PVP) both in AP and PVP. With the use of the optimal cut-off value of AP14.36and PVP20.63, a sensitivity and specificity of71%and99%were yielded,and the area ofROC were0.90and0.92in AP and PVP respectively. The mean ICs of GI (19.07±5.82, AP;24.66±6.55, PVP) was significantly higher (P=0.00,P=0.00) than those in NG (12.71± 2.13, AP;15.80±2.87, PVP)both in AP and PVP. With the use of the optimal cut-off valueof AP14.82and PVP19.52, a sensitivity and specificity of94%and77%were yielded, andthe area of ROC are0.86and0.89in AP and PVP respectively.Conclusion: Spectral CT quantitative analysis appears to be useful in characterizingmalignant gastric mucosal lesions, and might help increase the sensitivity and specificity ofgastric mucosal lesions detection.
