Abstract


Objective To explore the diagnostic value of the high-frequency ultrasound for knee cartilage degeneration, and with the results of arthroscopy were compared. Method 100 cases of trauma caused by the meniscus or ligament injury required arthroscopic examination of patients as research subjects,49 males and 51 cases female, aged 28-81 years, mean 54 years, preoperative ultrasound examination 1 week to observe the femoral condyle, within the lateral femoral condyle, pulleys midline ditch and medial slant, lateral cant corresponding parts of test results seen in line with the arthroscopic control analysis to the results of arthroscopy as the gold standard of diagnosis of cartilage degeneration of high frequency ultrasound sensitivity, specificity, accuracy, and analysis of diagnostic ultrasound and arthroscopic cartilage variable correlation and limits. Result This group of 100 patients with ultrasound a total of 500 femoral knee cartilage surface of high echo lines, deep high echo lines, changes in cartilage thickness of the internal echoes and a detailed observation of other indicators, the results except the performance of 247 normal articular cartilage of the remaining 253 outside a total of 290 articular cartilage lesions in different degrees, respectively, the central sulcus 40 for the pulley, inclined plane pulley 70 inside the central sulcus, the lateral incline 56,68 medial femoral condyle, femoral condyle 56. Ultrasonic phased as follows:Ⅰof 39,Ⅱof 101,Ⅲof 85,Ⅳof 65. In addition to the normal arthroscopic articular cartilage 210, another 290 detected a total of 327 articular cartilage lesions. Lesions distributed as follows: block 47 the central sulcus, central sulcus inside slant 68, the outer slope 57, femoral condyle 85, femoral condyle 70. Arthroscopic staging are as follows:Ⅰof 57,Ⅱof 86,Ⅲof 96,Ⅳof 88. By arthroscopy as the gold standard, false-positive ultrasound in this group 45 lesions, false negative 82. Using statistical software SPSS11.0, with 2×2 cross-classification table calculated sensitivity, specificity and accuracy, etc. Of ultrasound femur obtained articular cartilage degeneration of the sensitivity, specificity and accuracy were:trochlear groove median 74.4%,90.4%,82.0%; pulley inner slope 91.1%,77.1%,86.4%; lateral trochlear slope 80.7%,74.4%,78.0%; femoral condyle 67.0%,75.0%,69.7%; femoral condyle 64.2%,76.0%,68.9.8%. The overall sensitivity of 74.9% of cases, specificity 78.6%, accuracy 76.3%. Kappa coefficient evaluation using ultrasound and arthroscopy, by the SPSS statistical analysis, the Kappa coefficient of the ultrasound and arthroscopy was 0.46 (p<0.01), which is a medium consistency.Conclusion Ultrasound diagnosis of knee joint cartilage degeneration of high sensitivity and specificity, especially for the diagnosis of trochlear cartilage was significantly higher than the performance of internal and external femoral condyle; Ultrasound evaluation of cartilage degeneration stage and arthroscopic staging a good correlation can serve as a clinical diagnosis of knee joint cartilage degeneration important adjunct.
