Abstract
|
[Objective] To study cerebral infarction on serum homocysteine ??(Hcy), proton magnetic resonance spectroscopy (1H-MRS) and cerebral infarction, and between the two. Explore Hcy and 1H-MRS dysfunction in patients with cerebral infarction and prognostic value determined to make a definitive judgment, in order to facilitate its severity and prognosis in cerebral infarction, and to develop a reasonable treatment plan. [Method] within three days of the time of onset of 30 cases by the head MRI examination confirmed the diagnosis of cerebral infarction (arterial blood supply to the brain area) in patients with venous blood Hcy levels measured within 24 hours after admission, according Hcy levels, divided into normal group (≤ 15umol / L) 4, of mildly elevated group (15 20umol / L) increased 8 cases, moderately elevated group (20 ~ 25 umol / L) 10 cases of severe group (gt; 25umol / L) 8 cases; mirror of all patients with lesions in the central area of ??the contralateral normal brain tissues 1H-MRS analysis, the chemical shift of the main test object positioning, N-ethyl phthalate days aspartate (NAA) 2.0ppm, lactate (Lac ) 1.32ppm. The application of the U.S. National Institutes of Health Stroke Scale (NIHSS) 3 months after patient admission and treatment of neurologic impairment rating denoted as NIHSS1, NIHSS2. The value of the patient of Hcy NIHSS1, NIHSS2 and 1H-MRS metabolites's correlation analysis. [Results] The 1,1 H-MRS changes in cerebral infarction: (1) In addition to the 2 patients, 28 patients with infarction can be seen the Lac peak increased to varying degrees; contralateral mirror 3 patients there was a slight rise the Lac peak, the remaining 27 patients did not see the Lac peak. ⑵ In addition to the patients, 29 patients with infarction Center NAA peak compared with the contralateral mirror area have different degrees of decline, paired samples t-test confirmed ipsilateral NAA is lower than the contralateral mirror statistical significance (P lt; 0.001 ). Hcy elevated in patients with cerebral infarction (86.7%) and higher Hcy levels 1H-MRS in NAA values ??lower NAA values ??in each group pairwise comparisons were statistically significance (P lt; 0.001) . 3, on correlation analysis of various indicators: ⑴ of Hcy NIHSS1, NIHSS2 was positively correlated (r = 0.880 P lt; 0.01; r = 0.924 P lt; 0.01). The ⑵ infarct center NAA and NIHSS1 of NIHSS2 a negative correlation relationship (r = -0.842 P lt; 0.01; r = -0.818 P lt; 0.01). (3) infarct center Lac NIHSS1, NIHSS2 of was positively correlated (r = 0.776 P lt; 0.01; r = 0.796 P lt; 0.01). ⑷ Hcy NAA disease side was negatively correlated (r = -0.748, P lt; 0.01). Conclusion Hcy 1H-MRS NAA, Lac can be made of cerebral infarction and prognosis judgment, a statistically significant correlation between the two.
