Abstract


Objective To investigate the obstructive jaundice bile main ingredients of change and its correlation with bile duct injury, bile duct stones of recurrence to provide new ideas for clinical prevention. Establish the obstructive jaundice SD rat model, the determination of the obstruction is not relative level of jaundice, and grouped according to the the jaundice different level: A SO-IBD group: normal rats biliary-enteric drainage group, the control group; B, OJ24h-IBD C. groups: obstructive jaundice 24 hours - biliary-enteric drainage group; C OJ168h-IBD group: obstructive jaundice 168 hours - biliary-enteric drainage group. Observe the drainage of bile flow after bile TBA, gamma-GT, ALP, Tch phospholipid changes, pre-column derivatization reversed-phase high performance liquid chromatography bile bile salt component changes, TUNEL method biliary epithelial cells apoptosis observed morphological changes of the bile duct tissue analysis of bile BS / PL and bile duct injury severity score. Results obstructive jaundice after 168 hours in the C group bile hydrophilic bile salts taurocholic pig deoxycholic acid (THDCA) tauro ursodeoxycholic acid (TUDCA) significantly decreased in group A (p lt; 0.05); while hydrophobic bile salts taurocholic acid (TCA) taurocholic deoxycholic acid (TDCA), the the taurocholic CDCA (TCDCA), chenodeoxycholic acid (CDCA) was significantly higher in group C in group A (p lt ; 0.05); 2 open drainage, group B, group C THDCA, TUDCA significantly increased; TCA, TDCA, TCDCA, CDCA was significantly decreased. Internal drainage postoperative day total bile salt hydrophobic index (HI) C group than in group A and group B (p lt; 0.05), with the longer drainage time, HI gradually reduced to 14 days after three sets of HI and recovered to similar levels. HI elevated degree of obstruction time was positively correlated. After internal drainage of bile secretion in group C than in group A and group B; extension of time with the drainage, bile secretion gradually reduced; day of the group C BS / PL values ??significantly higher than that in the control group (p lt; 0.05 ), with drainage time growth, decreased progressively, close to the other two groups after 7 days. 4 linear regression analysis showed that the of postoperative bile γ-GT and ALP secretion and the secretion of bile salts are highly correlated. The histological assessment prompts bile duct injury severity score (BDISS) rats bile salt / phospholipid significant than low BDISS group. 6.C group of animals early postoperative biliary epithelial cell apoptosis rate was significantly higher than that in group A (P lt; 0.05). To gradually reduce the number of apoptotic cells of Group C and Group B to Group A after 7d are close to, there was no significant difference (P gt; 0.05). Conclusion: 1. Obstructive jaundice - biliary enteric drainage model more in line with the physical, has good stability and reliability than ideal animal model of obstructive jaundice, biliary; rats experienced prolonged OJ bile total bile salts in the increased proportion of hydrophobic bile salts, bile salt cytotoxicity;, OJ inhibit the secretion of phospholipids, liver, lack of micelles of phospholipids and bile salt formation, culminating in cytotoxic toxic bile, resulting in bile duct injury.
