Abstract


Chronic obstructive pulmonary disease (COPD) is an important health problem in the world . Each year more than 3 million people were killed , has become the fourth leading cause of death . Expected that by 2020 , COPD will be the following 3 cause of death after ischemic heart disease and stroke . The most COPD lead to the deaths occurred in patients with acute exacerbation of . Studies have shown that acute exacerbation of chronic obstructive pulmonary disease ( AECOPD ) 50 % to 70% caused by infection , 10% caused by environmental pollution , as well as more than 30% for unknown reasons . Vein thrombosis ( VTE ) can cause coughing and shortness of breath , pulmonary embolism ( PE ) is another common cause of COPD exacerbations . It is estimated that 25% of patients with AECOPD difficulty in breathing is caused due to the potential PE . PE or other venous thrombosis in patients with disease and COPD mortality is not twice the combined COPD . PE clinical manifestations such as dyspnea , tachycardia, pleural pain are non-specific . COPD patients with acute dyspnea is difficult to diagnose typical manifestations such as fever, sputum , wheezing and other tips acute exacerbation , when the absence of these symptoms , needs further examination to look for the underlying cause . COPD can lead to a variety of heart and lung diseases such as PE condition aggravated, or to cover up the symptoms of PE . PE and symptoms of AECOPD higher degree of overlap and are not specific , it is difficult to discover through the clinical features of COPD patients suffering from PE . COPD patients often have pulmonary ventilation and perfusion defects , resulting in lower specificity of noninvasive imaging diagnosis of PE . Recently with the application of the enhanced CT diagnosis of patients with COPD is a merger of the PE method can be the risk and discomfort to a minimum . Infection - induced acute exacerbation of COPD can be mitigated through effective treatment of antibiotics and corticosteroids , thromboembolism induced acute exacerbation of the need for effective anticoagulant therapy , resulting in prolonged treatment , and the prognosis is poor . AECOPD patients must take into account the possibility of PE , because untreated PE mortality up to 25% . For the diagnosis of patients with COPD , there must be a clear radiographic evidence found that the presence of thrombus to antithrombotic therapy . This review describes the clinical features of COPD patients with PE , and a more comprehensive set forth for diagnosis and treatment .
