Preliminary Study on Inflammation Factors of Exhaled Breath Condensate in OSAHS Patients and Assessment of Effects after CPAP Treatment

Abstract


Objective:To examine the inflammation factors in EBC of OSAHS patient,and evaluate the relation between the inflammation factors and the severity of these OSAHS patients.In addition,analyses the change of the inflammation factors in their EBC after CPAP therapy three months later;base on try to explore the lung inflammatory mechanism in patients of OSAHS and chronic intermittent hypoxemia.Methods:Thirty-nine patients were divided into three groups:normal grorp (AHI<5 times per hour),mild group(AHI between 5-20 times per hour);moderate and severe group(AHI>20 times per hour).we collect their EBC and analyses the inflammation factors of IL-6,IL-8,TNF-a in EBC.And we also collected the EBC of 9 patients in moderate and severe groups and after CPAP therapy three months later, test the three factors in their EBC.Results:(1)The concentration of IL-6 in normal group,mild group and moderate and severe group is respectively 45.131±19.547 pg/ml 67.361±9.226 pg/ml 88.892±15.314 pg/ml,there is significant difference between the normal group and mild group;there is significant difference between the mild group and moderate and severe group.(P<0.01).IL-6 is positively related to AHI,the coefficient of relation is 0.816,IL-6 negatively related to the lowest saturation of oxygen,the coefficient of relation is -0.796.(2) The concentration of IL-8 in normal group,mild group and moderate and severe group is respectively 49.502±35.482 pg/ml,39.330±14.530 pg/ml,50.365±18.3039 pg/ml.There is no significance between normal group and moderate and severe group.(P>0.05).(3) The concentration of TNF-a in normal group,mild group and moderate and severe group is respectively 25.500±12.958pg/ml,33.774±17.995pg/ml,50.416±24.621 pg/ml.There is significant difference between normal group and moderate and severe group(P<0.01).But there is no significant difference between normal group and mild group(P>0.05),no significant difference between mild group and severe group.(P>0.05)(4)The concentration of IL-6 in EBC in group after therapy of CPAP is significantly lower than group before therapy,49.6093±13.01287 pg/ml vs 88.8920±15.31386 pg/ml(P<0.01);The concentration of IL-8 in EBC in group after therapy of CPAP is no significant difference between the group before therapy 60.5765±25.42953 pg/ml vs 50.3652±18.30385(P>0.05);The concentration of TNF-a in EBC in group after therapy of CPAP is significantly lower than group before therapy 29.4970±25.20255 pg/ml vs 50.4161±24.62139 pg/ml (P<0.05).Conclusion:There is inflammatory reaction in patients of OSAHS due to the chronic hypoxemia.Especially in the EBC of these patients,there is significant changes in the concentration of IL-6 and TNF-a than controlled group,and there is significant reduction in the concentration in IL-6 and TNF-a after the therapy of CPAP.The result suggests we can evaluate the serious degree of OSAHS and monitor the effect of CPAP by the analysis of these inflammatory factors in EBC.
