Abstract

Objective: To evaluate Integrative the implementation of the results of the clinical pathway used in patients with lumbar disc herniation surgery, analysis of the implementation of all kinds of variations, to provide the basis for establishing convenient and feasible, in line with the actual clinical pathways, care of patients with treatment processes in order to regulate such ultimately reduce medical costs and improve the quality of medical care, increase patient satisfaction purposes. Method: 1. Formulate Integrative surgical treatment of lumbar disc herniation clinical path table; 2 to January 1, 2008 to 2008, the subsidiary of the Traditional Chinese Medicine Hospital of Luzhou Medical College hospital to lumbar disc herniation for the first diagnosis of patients, to determine the object of research for the control group (conventional group); January 1, 2009 to 2009, the subsidiary of the Traditional Chinese Medicine Hospital of Luzhou Medical College hospital to lumbar disc herniation as the first diagnosis of the patients determine the implementation of the study of the clinical pathway for Integrative Medicine Integrative Medicine clinical path (path group); 3, using descriptive analysis, t test, χ2 test, rank sum test statistical methods were compared in patients with total hospital days, preoperative the patient's average postoperative hospital stay, total hospital costs, in pharmaceutical utilization rate, clinical efficacy, patient satisfaction; 4, analysis of variation in Integrative Medicine clinical pathway; Results: Implementation of Integrative Medicine clinical pathway total hospital days before surgery, postoperative hospital stay were significantly shortened, the decline in the average cost of hospitalization, medical treatment rate increased clinical efficacy to improve patient satisfaction. 1 Integrative clinical pathway group total number of hospital days, preoperative hospital days and postoperative hospital stay compared to the traditional group were significantly reduce, which reduce the total number of hospital days, postoperative hospital stay. The results show that the Integrative Medicine in the implementation of the clinical path of lumbar disc herniation in patients with average length of hospital stay was 10.86 ± 2.08 days, 3.33 days less than the traditional group, the difference between the two groups was statistically significant (P lt; 0.01); in Integrative Medicine path group preoperative hospitalization days decreased 0.59 days compared to the traditional group, the difference was statistically significant (P = 0.026); Integrative path postoperative hospitalization days reduced compared to the traditional group 2.78 days, and traditional group the difference was statistically significant (P lt; 0.01). In hospitalization costs Integrative path group total hospital costs the western medicine costs than the traditional group were significantly reduced, with some increase in Chinese medicine costs, total hospital costs of traditional group 8059.21 ± 786.7, Integrative path group The total cost of hospitalization was 5817.69 ± 223.40, 2241.52 yuan lower than the traditional group by the t test was used to compare the difference was statistically significant (P lt; 0.01); western medicine costs Integrative path group than in the traditional group reduce Integrative path group 2172.71 ± 503.66, 1282.12 yuan less than the traditional group, the two groups were compared by t-test the difference was statistically significant (P lt; 0.01); Herbal fee by the traditional group 337.53 ± 63.56 liter to 391.48 ± 66.88 yuan; difference was statistically significant (P lt; 0.01) were compared by t test; clinical efficacy, traditional group of excellent was 68%, Integrative path group of excellent rate of 79 % Integrative path group is better than the traditional group and statistical significance (P = 0.036); aspects of patient satisfaction Integrative patient satisfaction path group (97.56 ± 3.29) than the traditional group (95.53 ± 4.11 a) higher patient satisfaction, and the difference was statistically significant (P = 0.005); Chinese medicine treatment rate, traditional medical treatment group was 83%, while the Integrative path group medical treatment was 96% two group difference was statistically significant (P = 0.022). The clinical path variation: 34 times variation occurs in the path of the implementation of the Traditional Chinese Medicine, 7 patients quit clinical pathway. Conclusion: 1. Chinese and Western medicine combined with clinical pathway shortened total hospital days, preoperative, postoperative hospitalization days; 2. Chinese and Western medicine combined with clinical pathway can reduce the total cost of hospitalization, to control the high cost, the increase in pharmaceutical treatment rate; 3. Chinese and Western Medicine clinical path can improve clinical outcomes, enhance patient satisfaction; 4. Integrative Medicine clinical pathway can save health care resources, and to ensure basic medical; 5. Integrative Medicine clinical path feasibility worth popularization and application in China.
