Abstract


Objective:To investigate breviscapine for the advanced primary open-angle glaucoma postoperative optic nerve protection of breviscapine injection, which provides the objective basis to the clinical glaucoma patients’optic nerve protection.Methods:Choose 40 advanced primary glaucoma patients (61 eyes) who was treatmented from February 2008 to November 2009 in Shenzhen Eye Hospital and had a glaucoma filtration surgery (trabeculectomy),and also the intraocular pressure of patients is controlled by less than 18mmHg two months later. Each group of patients who are examined by preoperative automatic static threshold perimetry are companied by varying degrees of glaucoma visual field defects.According to the extent of visual field defects, visual field defects will be divided into early stage、metaphase and advanced stage.15 patients (22 eyes) Among them was given 50mg breviscapine dissolved in 0.9% saline 250ml daily from the first day to the end for the treatment group,14days continuous medication and 7days withdrawal medication is a course of treatment, three consecutive treatments.15 cases of patients (23 eyes) after optic nerve did not given any optic never protective drug for the control group. The treatment group was composed of 9 eyes in metaphase and 13 eyes in advanced stage, The control group was composed of 10 eyes in metaphase and 13 eyes in advanced stage. Three One months later, compare the patients’blood pressure, pulse, visual acuity, intraocular pressure, vision, OCT results, analysis whether Breviscapus Injection can play a role in protecting the optic nerve, so as to provide objective basis to the optic nerve protection of primary glaucoma patients whose intraocular pressure has been clinically controlled.Results:The observation of 30 participant patients (45eyes) all lead the final analysis.①The comparison of the systolic blood pressure, diastolic blood pressure, stroke, intraocular pressure, C/D, visual acuity in control group and treatment group have no significant statistical difference (P> 0.05).②Comparison of the two groups before and after treatment view: After 30 days(two months) of treatment, the average field of vision defects of the treatment group patients in metaphase and advanced stage have changed. The MD Comparison of the Treated group patients in metaphase and advanced stage before and after treatment is analysised by paired t tests, the difference have statistically significant (P<0.05);. The MD comparison of the control group patients in metaphase and advanced stage before and after treatment is analysised by paired t tests, the difference have no statistically significant (P> 0.05);Being analysised by the independent sample t tests, the result of the observation group patients in metaphase and advanced compared with the control group after treatment have significantly different (P<0.05).③The comparison of OCT (RNFL) between two groups before and after treatment have no significant statistically significant (P> 0.05), The comparison of independent sample before and after treatment have no significant statistically differences (P> 0.05)④during treatment, the patient present symptoms of not significant adverse reaction, also of lack of the phenomenon of hemolysis and bleeding,etc.Conclusion:Breviscapine have an positive effect on the visual function of primary glaucoma patients in metaphase and advanced stage,and make a protective effect on glaucoma patients’visual function. Breviscapine make no effect on blood pressure, pulse, intraocular pressure, visual acuity, C/D ratio. Clinical studies show that, breviscapine can make a protective effect on the optic nerve damages of the primary open-angle glaucoma patients whose intraocular pressure is controlled.
