Abstract


Objective: To investigate the effects of cleft lip and palate occur factors, cleft lip and palate risk of discriminant analysis prediction model. Methods: A hospital-based 1:2 case-control study conducted a questionnaire survey of 113 cases of cleft lip and palate children with mothers and 226 cases of normal perinatal mother. Using the Epi Data3.0 data entry, SPSS13.0 for statistical analysis. Application of conditional logistic regression cleft lip and palate influencing factors were analyzed by univariate and multivariate analyzes, the selection of the univariate logistic regression analysis of significant variables using stepwise discriminant constructed Fisher linear discriminant prediction model. Results: Univariate logistic regression analysis showed that the low educational level of pregnant women in early pregnancy colds, the first trimester of pregnancy complications, pregnancy, birth control pills taken, before pregnancy or during pregnancy occupational exposure risk factors before pregnancy or during pregnancy housing decoration, early pregnancy often to eat kimchi smoked class keeping pets food before pregnancy or during pregnancy, husband suffering from chronic diseases, the husband's occupational exposure risk factors husband like to drink tea, low per-capita income, one of the spouses or both a cleft lip and palate family history, perinatal premature birth and low birth weight is a cleft lip and palate The risk factors; premarital medical examination, early pregnancy regular consumption of fish and meat and eggs, vegetables, fruits and milk soy milk protective factors in cleft lip and palate. Multivariate conditional logistic regression analysis showed that: cleft lip and palate occur early pregnancy colds (OR = 2.395), occupational exposure risk factors for pre-pregnancy or during pregnancy (OR = 22.018), early pregnancy often drink milk, soy milk (OR = 0.167), one of the spouses or both a cleft lip and palate family history (OR = 20.817), husband suffering from chronic diseases (OR = 17.677), low annual per capita household income (OR = 3.785), a perinatal low birth weight (OR = 25.532). Into the cleft lip and palate discriminant analysis and forecasting model variables including maternal occupational exposure to risk factors (X1), pre-marital medical check (X2), taking pre-pregnancy contraceptives (X3), before pregnancy or during pregnancy housing decoration (X4) of early pregnancy to drink milk, soy milk (X5 ), the husband of occupational exposure risk factors (X6), one or both spouses family history (X7) and annual household per capita income (X8). Cleft lip and palate discriminant analysis prediction model function formula: Z = 0.366 X1-0.237 X2 0.239 X3 0.494 X40.297 X5 0.281 X6 0.332X7 0.325X8. Use of self-validation, jackknife validation and ROC curve analysis to verify the discriminant analysis prediction model, self-validation accuracy of 77.6%, sensitivity 62.8%, specificity 85.0%; the jackknife validation accuracy of 77.3% sensitivity 61.9%, specificity 85.0%; discriminant analysis and forecasting models under the ROC curve area of ??0.796. Conclusion: Genetic factors are important risk factors for fetal cleft lip and palate. In addition, family economic status, maternal occupational exposure to harmful substances, the early pregnancy nutritional status, husband's health status and environmental factors such as fetal cleft lip and palate occur. Discriminant analysis prediction model can predict the occurrence of cleft lip and palate birth defects from the epidemiological point of view.
