Purposes: (1) to explore the application of RT-PCR Technique to Detect the specificity and sensitivity of the peripheral blood of patients with esophageal cancer CEAmRNA, CK19mRNA expression. (2) to investigate the relationship between the peripheral blood in expression of CEAmRNA, CK19mRNA, clinical and pathological data collected follow-up data, The evaluation combined detection of peripheral blood CEAmRNA, CK19mRNA expression value to predict prognosis of esophageal cancer. Materials and methods: (1) human esophageal cancer cell lines Ec-109 with known concentrations of normal human peripheral blood mononuclear cells mixed adjustment line CEAmRNA, CK19mRNA RT-PCR reaction for different orders of magnitude concentration, respectively, to detect RT- The sensitivity of the PCR method, and as a positive control. (2) collection of Shantou University Medical College Affiliated Tumor Hospital from February 2009 to May 2009, esophageal cancer surgery 57 patients the day before surgery to extract fresh peripheral blood (heparin) 10ml, the first postoperative day using the same method of pumping 10 ml of blood, human lymphocyte separation the liquid extraction mononuclear cell layer detect where CEAmRNA, CK19mRNA of expression by RT-PCR method. 18 healthy people detected with the same method, lung or benign esophageal patients with peripheral blood CEAmRNA, CK19mRNA as negative controls. (3) data collection and follow-up data were statistically analyzed to evaluate the combined detection of peripheral blood CEAmRNA in CK19mRNA expression and clinical factors and prognosis of patients with esophageal cancer relationship. Results: RT-PCR detection in peripheral blood of patients with esophageal cancer CEAmRNA in sensitivity are 106 CK19mRNA normal peripheral blood cells can be detected in an expression cancer cells CEAmRNA, CK19mRNA. The 18 cases donor peripheral blood CEAmRNA, CK19mRNA were negative. Single-gene test results and the correlation of the clinical and pathological data of 57 cases of esophageal cancer in peripheral blood of patients in the preoperative group 54.4% of the the 31 cases CEAmRNA positive rate, the 17 cases CK19mRNA positive rate was 29.8%; postoperative group of 6 patients positive CEAmRNA, the positive rate was 10.5%, two cases CK19mRNA positive rate was 3.5%. Preoperative peripheral blood of patients with esophageal cancer CEAmRNA in CK19mRNA expression were correlated with tumor depth of invasion (pT), lymph node metastasis (pN) and pathological stage (pTNM stage) on (P lt; 0.05 and P LT; 0.01) Group after the peripheral blood of patients with esophageal cancer CEAmRNA in CK19mRNA expression were correlated with tumor depth of invasion (pT) about (P lt; 0.05 and P LT; 0.01). No correlation in the patient's age, sex, tumor site, the length of cell differentiation and single gene detection result. Multi-gene test results with clinical and pathological data of this study, the simultaneous detection of CEAmRNA CK19mRNA genes, at least one genetic test results positive (CK19/CEA), preoperative group expression rate was 61.4% (35/57 ), and the depth of tumor invasion, TNM staging (P lt; 0.01); postoperative group expression rate was 12.3% (7/57), has a correlation with the depth of tumor invasion (P lt; 0.01) 13 cases of preoperative gene expression (22.8%) for patients with advanced cancer, statistics show that with the depth of tumor invasion, lymph node metastasis, TNM stage correlation (P lt; 0.05); 1 cases after two gene positive expression of both (1.8%) patients with pT4N1M0, statistics show that its correlation with the depth of tumor invasion (P lt; 0.01); patient's age, sex, tumor location, length, cell differentiation and genetic testing results There was no correlation. The statistical results also showed that preoperative group CEAmRNA, CK19mRNA gene expression positively correlated (P lt; 0.05). Follow-up results of 57 cases of esophageal cancer patients were followed up to May 2010, lost to follow in 4 cases, 7 died, the follow-up rate of 93.0%, 7 deaths in patients with a median survival of 8 months, 57 patients with a median follow-up time of 13 months, 1 year survival rate was 87.7%. Peripheral blood patients with CEAmRNA positive death preoperative 4 patients (4/7, 57.1%), the postoperative group 1 patients (1/7, 14.3%); the peripheral blood CK19mRNA positive patients death preoperative group 2 patients (2/7, 28.6%), postoperative group 1 patients (1/7, 14.3%). Both preoperative positive patients, 2 patients died (2/7, 28.6%). Preoperative and postoperative indicators are negative patients, 3 patients died (3/7, 42.9%). Univariate analysis of preoperative, postoperative of CEAmRNA, CK19mRNA expression on the prognosis of patients, results not statistically significant. The rest of the clinical factors using logistic regression analysis, patients with esophageal cancer clinical factors and peripheral blood CEAmRNA, CK19mRNA, the expression on the prognosis of patients, the results showed: lymph node metastasis in patients with risk factors for prognosis, P = 0.001; prognosis of esophageal no significantly affect the result was not statistically significant. Multiple variables indicators were used Kaplan-Meier analysis and Log-rank rank test, the results showed: no lymph node metastasis in esophageal cancer patients with postoperative survival rate is higher than with lymph node metastasis, the difference was significant (P lt; 0.01); other clinical factors on survival is not obvious, the difference was not statistically significant (P gt; 0.05) Conclusion: RT-PCR method combined detection of peripheral blood of patients with esophageal cancer CEAmRNA, CK19mRNA expression with high sensitivity and specificity can be used as an effective method of esophageal cancer micrometastasis. Peripheral blood of patients with esophageal cancer CEAmRNA in CK19mRNA expression and tumor depth of invasion (pT), lymph node metastasis (pN) and pathological stage (pTNM staging), and patients survival rate was no significant correlation. Surgical removal of the tumor, the peripheral blood of patients CEAmRNA in CK19mRNA positive rate decreased specific mechanism needs further study.
