Abstract


Objective : To investigate the use Oxiracetam injection treatment of the acute phase of severe head injury patients , apolipoprotein E (APOE) gene polymorphisms affect the efficacy of the drug . Methods: 78 cases of acute period , cases of severe traumatic brain injury , venous blood 2 ml of each patient , genomic DNA was extracted using the multiplex polymerase chain - high temperature ligase detection reaction (PCR-LDR) to detect the APOE gene single nucleotide polymorphism; every object within 48 hours after admission to the injection in the treatment of Oxiracetam week , during treatment, prohibit the use of other nootropics . GCS scores before and after treatment (Glasgow coma scale), MMSE score (Mini mental state examination) and NOS-TBI score (Neurological outcome scale for traumatic brain injury), to analyze the relationship between the different subtypes and therapeutic effect of the APOE gene . Results : 78 patients detected ε2 / 2 , ε3 / 3 , £ 4 /4 three kinds of homozygotes and £ 2 / £ 3/4 two heterozygous Not Detected £ 2 /4 heterozygotes £ 3 / 3 homozygotes highest proportion ε2 / 2 homozygotes the lowest proportion of APOE phenotype ; allele frequencies by £ 3 the highest , accounting for 80.13% . The £ 2/2 type and type ε2 / 3 together as a group , £ 3/4 type and £ 4 / 4 type together as a group , then together for comparison with the type ε3 / 3 before and after treatment in each group GCS , the MMSE score significantly improved , NOS-TBI has significantly reduced . £ 2/2 £ 2 / 3 and £ 3 /3 type between efficacy was no significant difference , £ 3/4 ε4 / 4 type with more than two groups were differences between groups . Conclusion: ① Oxiracetam injection of the acute phase of an effective treatment of severe traumatic brain injury . The ② apolipoprotein E gene polymorphism the Oxiracetam injection treatment of acute period, the effect of severe traumatic brain injury influential . ( 3 ) carrying the APOE £ 4 allele in patients with Oxiracetam injection treatment effect than the non- poor patients carrying the genotype . APOEε4 allele may be one of the reasons for this result .
