Duration of Anticoagulant Therapy after Venous Thromboembolism: A Meta-Ananlysis

Objective: To evaluate the treatment of venous thromboembolism in patients with anticoagulant therapy . Methods: The system retrieves the Chinese Journal Full-text database , Chinese Biomedical Database , VIP database . Database , the Cochrane Library , PubMed database , retrieval time building a database to March 2011 , and check the references included in the study to supplement . Into all randomized controlled trials of long-term anticoagulant therapy for patients with venous thromboembolism . The risk of bias assessment tool using the Cochrane handbook 5.0 recommended methodological quality of included studies . Application RevMan 5.0 software included in the study results , the test for heterogeneity , a joint study using a fixed effects model or random effects model to calculate pooled odds ratio (OR) and its 95% CI, draw a funnel plot to discuss publication bias . Results : A total of six randomized controlled trials . Meta-analysis showed embolism recurrence rate of venous thromboembolism in patients receiving anticoagulant group of six months or longer anticoagulant group and 3 months showed no statistically significant [ OR 0.78 , 95 % CI ( 0.49 , 1.24 ) ; P = 0.30 ] , two sets of incidence of bleeding was no significant difference [ OR 2.79 , 95 % CI ( 0.93 , 8.36 ) ; P = 0.07 ] . Subgroup analysis : idiopathic VTE accept the recurrence rate of 12 months or longer anticoagulant anticoagulant three groups showed no statistically significant [ OR 0.26 , 95 % CI ( 0.01 , 5.77 ) ; P = 0.39 ] , but longer anticoagulation bleeding rate in the short - term treatment group [ OR 4.64 , 95 % CI ( 1.31 , 16.35 ) ; P = 0.02 ] . Conclusion : Extension of anticoagulation time did not significantly for patients with venous thromboembolism benefit from adequate anticoagulation therapy may be 3 months ; to extend anticoagulation may increase the risk of bleeding .
