Abstract

As a malignant tumor which threats human’s health seriously,the mortality and morbility of lung cancer take the top stop in most countries. According to the prediction of WHO, dead cases induced by lung cancer will exceed one million every year in China.In period of first visit to hospital,80% patients have missed the best occasion of synthetic radical treatment or suigery. Because of this, it is importianto study the molecular biology mechanism of metastasis and to find the way of molecular targeted therapy.Metastatis refers to the interreaction of tumor cell and organism cell.The key approach is cell adhesion.P-slectin effects the initiative adhesion and transference, which mediate the unite of turner cell and platelet, meaning the formation of tumor embolus,and the transferring ability of tumor boosts.CD24 is The ligand of P-slectin .It can accelerate the progress of multiplication ,discent and metastasis of tumor.This time we study the effection of P-slectin and CD24in the metastasis of lung cancer.Method:We selected 70 patients with lung cancer who visit in thoracic surgical department of China-Japan Union Hospital Affiliated Jilin University during a period from 1995 to 2009 as the research objects studied.Among them,34 patients suffers from adenocarcinoma while 29 patients suffers from aquuamous cell carcinoma (SCC).7 patients suffers from small cell lung cancer. After that,we analyse the relativity of P-SLT/CD24 and the data of clinical and pathologic situations and prognosis.Results:1 The expression of P-slectin in lung cancer(1)The positive expression in tumor can be observed in vessel endothelial cells of mesenchyma, which is brown madder or tawny. It can be seen in the mesenchyma of arteriole,venule, small artery and small vein of SCC and adenocarcinoma,while it can not be seen in the 7 small cell lung cancer(SCLC).But because the amount of SCLC is small,the confidence level need to be testified.(2)The positive rate of P-slectin in NSCLC is 39.68%(25/63).The rate in SCC is 13.79%,and it in adenocarcinoma is 61.76%(21/34) which is higher than that in SCC.The difference is statistical significant(P<0.05).(3)The positive rate in stageⅠandⅡis 20% and in stageⅢis 64.28% (18/28).The difference is statistical significant(P<0.05). (4)In NSCLC ,the expression of P-slectin is independent with age, sex and differentiation grade of tumor. The difference is statistical insignificant.2. The expression of CD24 in lung cancer(1)The positive expression can be observed in cell membrane and cytoplasm while can not be seen in nucleus. cell membrane and cytoplasm are brown madder or tawny in SCC and adenocarcinoma.And it is negative in the 7 SCLC. But because the amount of SCLC is small,the confidence level need to be testified.(2)The positive rate of CD24 in NSCLC is 41.3%(26/63). The rate in SCC is 20.7%(6/29),and it in adenocarcinoma is 58.8%(20/34) which is higher than that in SCC.The difference is statistical significant(P<0.05).(3)In NSCLC,the expression of CD24 is independent with age, sex, clinical stage and differentiation grade of tumor. The difference is statistical insignificant.3 The expression of P-slectin and CD24 and the survival time of lung cancer(1) The mean survival time of patients of whom the expression of P-slectin is positive is 38.5±5.3months. while that of negative is70.3±6.5 months. The difference is statistical insignificant.(2) The mean survival time of patients of whom the expression of CD24 is positive is 38.2±8.3months,while that of negative is70.5±6.5 months. The difference is statistical insignificant.To sum up, we can get the below conclusion.According to the TNM staging, the expression of P-slectin in stageⅠandⅡis 20% and in stageⅢ,suggesting P-slectin correlates with the TNM staging. In NSCLC,the expression of P-slectin and CD24 in adenocarcinoma is higher than that in SCC,suggesting P-slectin and CD24correlates with the character of adenocarcinoma that the hematogenous metastasis of adenocarcinoma occurs earlier. The mean survival time of patients of whom the expression of P-slectin and CD24 is positive is shorter than that expression negative, suggesting P-slectin and CD24 are propitious to tumor’malignant act.So it is feasible to judge the pathological classification,property,metastasis ability and prognosis, and to treat carcinoma targeting P-slectin and CD24.
