Retrospective Study of Adjacent Segment Degeneration of the Upper Level to a Lumbar Fusion with and without Instrumentation

Abstract


Research Objective: To compare the clinical lumbar spine after single segment adjacent on the difference between a segment degeneration , including the fusion group and non- fixation group . Method : select a total of 56 cases of patients with disc herniation of lumbar 4-5 , I set of 30 routine lumbar 4-5 level posterior laminectomy disk resection , interbody fusion and pedicle screw fixation surgery ; Ⅱ group 26 routine lumbar 4-5 level posterior laminectomy , disk excision , wearing 6 months after surgery waist ; two groups of patients preoperative lumbar MRI confirm their withdrawal disc in the adjacent segment change in post-operative follow up for 3 years , lumbar MRI follow-up , postoperative adjacent segment degeneration (ASD adjacent segment degeneration) degree Pfirrmann grading system grading and assessment , and for statistical analysis. Follow-up results : patients lose contact patients died , 2 patients in remote mountainous areas , 4 patients unfused (not a fixed group of patients ) , 3 patients failed to provide MRI imaging data , so the total of the remaining 44 patients Ⅰ 24 cases , group II , 20 cases . The remaining 44 patients the preoperative and postoperative radiographic follow-up data . Results : of fixation devices fusion patients lumbar single segment of ASD group than in the non- fixation devices group were significantly increased , and lumbar spine MRI is a reliable method for the diagnosis of ASD . Conclusion: a single segment in the lumbar spine surgery , the use of pedicle screw fixation devices significantly heavier one on the neighboring segment degeneration , but do not use the internal fixation devices easy to form lumbar instability affect postoperative healing effect , it preoperative response adjacent segment disc fully assessed to select the appropriate procedure . 2, a single segment in the lumbar spine surgery , during surgery to protect the adjacent segments of the composite structure and maintain the normal physiological curvature of the lumbar fusion segment for it is essential to avoid the formation of ASD . 3 , in the in many lumbar single segment postoperative patients , although there are imaging the ASD , but often has no clinical symptoms . 4 , aging, women , and other factors is very important risk factor for the formation of ASD now followed up with lumbar MRI confirmed relative to the secondary surgical risk factors in previous studies . 5 lumbar MRI in clinical studies for the diagnosis of clinical ASD really is a non-invasive and more accurate measures .
