Abstract


Background and Purpose : total body weight is usually as a standard to guide clinical anesthesia and intravenous induction dose , but most intravenous anesthetic induction drug can accumulate in the fat , leading to the overdose of obese patients . This paper aims to observe minor surgery fat weight intravenous anesthetic induction dose after induction loop system stability and postoperative recovery time , explore fat weight with general anesthesia, intravenous induction dose related . Methods: 150 cases of elective surgery under general anesthesia patients , ASA Ⅱ - Ⅲ grade , age 20-65 years old , were divided into five groups according to body mass index (BMI) , that the standard weight group ( I group ) ( BMI18.5 - 23.9) , partial fat group (Ⅱ group ) ( BMI24.0 - 26.9 ) , obese group (Ⅲ group ) ( BMI27.0 - 29.9 ) , severe obesity group ( IV group ) (BMI30.0-35.0) and very severe obesity group (V group ) (BMI ≥ 35.0 ) were randomly divided into Ⅰ - Ⅴ groups each group the fat weight administered group (A ) , total body weight administered group (B ) and standard weight dose group (C group). Fat-free mass measurement method : body composition analyzer to measure the direct determination of the parameters for the body resistance . The analyzer according to the subjects' age, sex, height , weight and other parameters to calculate the lean body weight of the subjects . : Induction of anesthesia, followed by intravenous injection of midazolam 0.04mg/kg, vecuronium 0.1mg/kg propofol 2mg/kg, sufentanil 0.5ug/kg . The surgery using inhalants maintain longer additional intravenous anesthetic . Observed after induction of anesthesia depth and awakening time after the end of anesthesia . Results : group Ⅰ and group Ⅱ induced depth of anesthesia and anesthesia after waking time were not statistically different , but the III , IV - V group group after anesthesia induction depth sorting for group B , group A , C group , but between group A and group C showed no statistically significant difference between group B and group A , C group results were statistically significant ; postoperative recovery time in group C gt ; A group gt ; B group , C group , group A no significant difference , but in group C and group B , group A and group B differences were statistically significant . Differences and anesthesia group after the induction of anesthesia depth after the marked degree of awake time differences are Ⅴ group gt ; the group Ⅳ gt; Ⅲ group . Conclusion : Obese patients intravenous anesthetic induction dose fat weight as the standard calculation is more accurate , and the degree of accuracy increases with increasing body mass index BMI .
