Abstract


Objective: To compare optical cable guide tube and the ordinary direct laryngoscopy boot intubation elderly surgical patients hemodynamics and stress hormones. Methods: ASA Ⅰ ~ Ⅱ grade, aged 60 to 75 years old, weighing 45 to 75kg, height 150-175 cm, anesthesia in elderly patients undergoing elective surgery were a total of 40 cases were randomly divided into two groups: direct laryngoscopy group (A ) and optical cable guide tube group (B), 20 cases each. Group A: During the induction of general anesthesia with direct laryngoscopy guide the endotracheal tube into the trachea; B Group: optical cable guide tube to guide the endotracheal tube into the trachea during induction of general anesthesia. Two different groups in different ways intubation time point were recorded SBP, DBP, HR; extract the outer periphery of the two groups of patients at different time points arterial blood 3ml, left in the centrifuge tube (containing heparin), centrifuged (3000r/min, 5 min) The resulting plasma stored at -80 ° C, within 4 months by radioimmunoassay (RIA) determination of epinephrine and norepinephrine concentrations. Results: the general situation of the patients were age, sex, weight, height, intubation time, the difference was not statistically significant (P gt; 0.05) Compared with group B, A group placed endotracheal tube after each point in time systolic blood pressure and plasma epinephrine and norepinephrine epinephrine concentration was significantly higher (P lt; 0.05), 30 seconds diastolic blood pressure and heart rate rose significantly after intubation difference was statistically significant (P lt; 0.05), 5 minutes after intubation two sets of diastolic blood pressure and heart rate difference was not statistically significant (P GT; 0.05); compared with before intubation, the two groups after intubation 30 seconds plasma epinephrine and norepinephrine epinephrine concentrations were significantly higher, the difference was statistically significant (P lt; 0.05), 5-minute group B plasma epinephrine and norepinephrine concentrations was no significant difference compared with before intubation (P gt; 0.05) A plasma epinephrine and norepinephrine concentration before intubation difference was statistically significant (P lt; 0.05); comparison group A, 30 seconds, systolic and diastolic blood pressure than before intubation significantly increased, the difference was statistically significant (P lt; 0.05); B group 5 minutes systolic and diastolic blood pressure than before intubation decreased significantly compared, the difference was statistically significant (P lt; 0.05); A group intubation 30-second heart rate more than before intubation was significantly increased, the difference was statistically significant (P lt; 0.05); remaining at each time point systolic blood pressure, diastolic blood pressure, heart rate, no significant difference was no significant difference (P GT; 0.05). Conclusion: optical cable guide tube assisted intubation does not reduce the difficulty of endotracheal intubation, compared with ordinary direct laryngoscopy assisted tracheal intubation compared caused by the patient's hemodynamic changes and catecholamines in vivo release of stress hormones moderate, the patient underwent endotracheal intubation during the stress response can be significantly reduced.
