Objective To investigate the plasma aldosterone levels (PA) with metabolic syndrome (MS) and the relationship of the components in the elderly population, and to provide a basis for further study of the pathogenesis of MS. Methods A cross-sectional population-based study in Qinghe County, Hebei Province in 2009 using cluster sampling method villages extraction rural elderly people over the age of 70 as an object of study, drawn from 320 villages the 88 villages baseline data survey, should check number 2289, the actual surveyed 2014 people, a response rate of 88.0%. With complete data of a total of 1969 people, including 976 men, 993 women; mean age of 73.76 ± 3.78 years. Using standard epidemiological survey methodology, the survey included a questionnaire, physical examination, and laboratory tests. Survey include: general survey, past history, and habits; physical examination including: height, weight, waist circumference, blood pressure, etc.; laboratory tests: routine biochemical tests, radioimmunoassay measurement of plasma PA renin activity (RA), insulin (INS); France Artech-Medical Company to develop and produce of CommScope music (Complior SP) detection systems detect carotid - femoral pulse wave velocity (cfPWV). According issued by the International Diabetes Federation (International Diabetes Federation, IDF) in April 2005, the definition of the MS, the follow-up crowd in the MS group and the non-MS group, PA levels were compared, and the analysis of the PA and MS components and cardiovascular disease. Measurement data described Mean ± SD, the two groups were compared using analysis of variance, and compared by χ2 test, and multivariate analysis using logistic regression and partial correlation analysis. Results ① The complete data of the study population of 1969 people, including 976 men, 993 women, average age 73.76 ± 3.78 years. The median level of plasma PA 0.18ng/ml. Population, the prevalence of the metabolic syndrome was 30.9%. PA level compared with non-MS group the elevated MS group, grouped by aldosterone tertile (cut-point value of 0.15 and 0.22ng / L) with PA level rise, increased prevalence of MS (prevalence 24.4 ℅, 31.8 ℅, 37.7 ℅). ② partial correlation analysis using partial correlation analysis method to control the effects of age and gender, results show that plasma aldosterone levels and waist circumference (WC) (r = 0.092, P = 0.000), triglycerides (TG) (r = 0.102, P = 0.000), fasting plasma glucose (FPG) (r = 0.109, P = 0.001), diastolic blood pressure (DBP) (r = 0.053, P = 0.019), body mass index (BMI) (r = 0.60 p = 0.000), total cholesterol / high-density lipoprotein cholesterol level ratio (TC / HDL-C) (r = 0.09p = 0 .. 000) showed a significant positive correlation with high density lipoprotein cholesterol level (HDL-C) (r = -0.03, P = 0.64), systolic blood pressure (SBP) (r = 0.04, P = 0.104) was not statistically significant. ③ PA tertile grouping (cut point value of 0.15 and 0.22 ng / L), after adjusting for age and gender, WC, insulin resistance index (Homa IR), TG, FPG, DBP, TC / HDL-C, BMI number average with elevated aldosterone levels and increased (P lt; 0.05). ④ MS non-conditional logistic regression analysis showed that, controlling for gender, age the the neck femoral pulse wave velocity (cfpwv), smoking status, drinking status, family history, with the rise of aldosterone quantile, MS dangerous (OR2 = 1.41, 95% CI1.07 ~ 1.86; the OR3 quintile = 1.87, 95% CI1.42 ~ 2.44). ⑤ The PA, the renin (RA), and plasma PA and RA activity (ARR) tertile, with the PA, RA level elevated MS and diabetes prevalence rates increase, but increases with the ARR level, diabetes prevalence decreased; with elevated RA level, lower prevalence of hypertension, elevated prevalence of hypertension increases with ARR level. Conclusion: PA in the elderly population, with most of the components of the Homa IR and MS; PA with MS has a significant correlation between high PA levels of MS risk factors; PA, RA occur in MS and related cardiovascular diseases play a role, participation mechanisms need further study
