Thrombocytopenia in addition to the primary reduction, or a secondary symptom of many diseases in clinical cause serious adverse consequences. In recent years, has become one of the important means of treatment of thrombocytopenia due to of transfusion technology development, clinical apheresis platelet transfusion. However, with the increase in the number of infusions, some chronic, long-term infusion patients platelet transfusion refractoriness (refractoriness to platelet transfusion, PTR) phenomenon this issue in the clinical increasingly prominent has become a very difficult problem, causing the blood transfusion industry concern. The platelet transfusion invalid for many reasons, immune factors accounted for a small part but caused clinical consequences are very serious. Immune factors on human leukocyte antigen (human normal renal function antigen of HLA) and human platelet antigens HPA (human platelet antigen, HPA) is relatively mature, to bring the gospel to solve the PTR, the last few years CD36 antigen into people vision, immune factors-born. CD36 antigen in the country is relatively small. Thrombocytopenia will affect CD36 antigen expression? Deletion of CD36 antigen mainly and gene mutation? Lack of CD36 antigen gene mutations and abroad are the same? Deletion rates are the same? Presage of CD36 antigen is missing and platelet transfusion refractoriness ? these problems are the blank areas of the domestic research. To solve these problems, we have adopted a PCR-SSP (sequence-specific primers polymerase chain reaction) method in our hospital 141 cases of thrombocytopenia patients and 300 normal antigen CD36 gene mutation detection, the results show that, in our study there is a certain lack of CD36 antigen in the specimen, and the absence of rate no difference between patients and normal thrombocytopenia therefore thrombocytopenia does not affect the CD36 antigen expression of CD36 antigen missing is caused by a mutation in its mutation point differences with Europe and the United States population. In patients with platelet infusions, we carried out the statistics the platelet transfusion invalid cases, detection of platelet antibodies (including HLA and HPA) statistics CD36 antigen missing number, the results so we have reason to suspect that the platelet infusion invalid phenomenon occurred and CD36 antigen is missing, perhaps future clinical diagnosis of CD36 antigen missing, you can become a routine examination before the platelet transfusion, thereby matching infusion, to prevent the occurrence of PTR.
