Abstract


Objective To explore the influencing factors of rural maternal deaths occurred in Yunnan Province , to provide a reference for the development of effective intervention strategies and measures to further reduce the maternal mortality rate . 1:2 case-control study of influencing factors of maternal deaths occur . From June 2009 , according to information reported by the Yunnan maternal mortality monitoring network , select pregnant women died within 42 days after childbirth ≥ 28 weeks of pregnancy , 100 cases of maternal death family except for the cause of the accident , in the same administrative village or neighboring villages , 1:2 matched control group in accordance with the criteria of the family structure , area of ??residence , family economic situation . Using a self- designed questionnaire , households face-to-face surveys . Apply conditional logistic regression model to explore the influencing factors of maternal deaths occur . Results of maternal deaths constitute the top three causes of death obstetric hemorrhage , amniotic fluid embolism , gestational hypertension , the constituent ratio of 48.0% , 19.0% , 13.0% , respectively . Basic level of control over the economy and residence the maternal mortality factors Logistc single factor analysis showed that : maternal education , maternal age , husband , age , maternal the number of children in the pre- pregnancy prenatal examinations , whether done the number of prenatal examinations , prenatal check abnormal location of delivery , mode of delivery , time from home to hospital and maternal mortality occurred statistically associated . Stepwise regression fitting main effect equation , multivariate conditional logistic regression analysis showed that maternal older (OR = 1.109), not done the prenatal check ( OR = 9.165 ) , cesarean section ( OR = 4.883 ) , a long time from home to hospital (OR = 1.043) risk factors for maternal deaths occurred . Rural maternal mortality in the conclusions of this study are the leading cause of postpartum hemorrhage . Rural maternal deaths occur with childbirth and aging , health services and poor health unit technical level is not high on . Tip to promote equal access to maternal and child health services in rural areas , to strengthen maternal high-risk screening and management , to improve the quality of health units obstetric , can reduce the occurrence of maternal mortality .
