Abstract


Objective:Gastric carcinoma is the most common malignant tumor and account for first in the malignant tumor of digestive tract. The incidence of gastric cancer exists regional difference. The present study aimed to investigate prevalence and clinical characteristics of gastric carcinoma.Methods:The patients, who underwent gastroscopy in our hospital due to dyspepsia during November 2004 to October 2010, were retrospectively analyzed. The prevalence of gastric carcinoma and the effects of age and sex on it were investigated.Results:Total 64943 patients were evaluated.648 patients were diagnosed as gastric carcinoma by gastroscopy and pathology, male 446, female 202 cases. The prevalence of gastric carcinoma in patients with dyspepsia was 1%. The age of patients with gastric carcinoma was 16-84 years old, the average age was 58.4 years. Among 648 patients, young people under 40 years old was 63 cases (9.72%), old people over 40 years was 585 cases (90.28%). The incidence of gastric carcinoma increased with age(r=0.234). The age of most patients with gastric carcinoma was 61-70 years. Before 40 years old, there was no significant difference in the prevalence of gastric carcinoma between male and female (P> 0.05). However, after 40 years old, the prevalence of gastric carcinoma in male was markedly higher than female (P<0.05). During November 2004 to October 2007, detection rate of gastric carcinoma by gastroscopy was 1.06%(268/25067, and the detection rate was 0.93% (380/39876) during November 2007 to October 2011. There was no significant change in detection of gastric carcinoma in the past 6 years (p>0.05). Antrum is the main site of gastric cancer (49.9%), fundus or cardia carcinoma (14.5%), and gastric body carcinoma(35.6%). Poorly differentiated type of gastric adenocarcinoma was highest (88.1%). The most common morphologic type is Bormann III and II type (73.1%). H.pylori detection rate was 40.1% in patients with gastric carcinoma.Conclusion:The incidence of gastric carcinoma increases with age and the incidence of gastric carcinoma in male is higher than that in female. Poorly differentiated adenocarcinoma is the most commom pathologic type. Antrum is the main site of gastric carcinoma. The prevalence of gastric carcinoma is probability associated with H.pylori infection.
