Abstract


Objective: To observe the histological and imageological change of the joint after the establishment of rabbit model of TMJ with radiotherapy.Methods: 63 New Zealand rabbits were assigned to radiotherapy group and untreated group. We employed the untreated group as blank control which normal TMJ data was collected and no intervention given. The animal subjects in radiotherapy were further randomly assigned to 6 groups with various doses of radiotherapy and the establishment of rabbit model of TMJ with radiotherapy was performed. The doses of radiotherapy for 6 groups were 10.8, 16.2, 21.6, 27, 32.4 and 37.8 Gy. The execution for 3 animal subjects in every group with radiotherapy was performed after 1, 3 and 6 month after initial exposure and the MRI and histopathological technique were applied.Result: 1. The rabbit model of TMJ with radiotherapy were successfully established and all animal subjects were survived to the observation time point. 2. The histological result of animal subjects with radiotherapy indicated that the osteoporosis and hyaline degeneration in condylar cartilage, boarded interval between fibers, deformability and disarrangement of proliferative cells, various instensity of thinning in proliferative zone , hypertrophic zone and calcified cartilage zone, and reduction in cell counts. As can be seen in articular disc, the interval between fibers were boarded and fissure detected. Local aggregation of chondrocyte can be identified inside the joint disk. The histological change of joint with radiotherapy was positively associated with the dose and time of radiotherapy. The destruction of joint in F group can be qualified as the most severe among all radiotherapy groups and 6 months group was more severe than 1 month and 3 months groups. 3. The imageological change of TMJ with radiotherapy: No abnormal signal detected by applying MRI among animal subjects after A-F groups after 1 month of radiotherapy. After 3 months of radiotherapy, abnormal signal obtained in TMJ among animal subjects in D group, and the range of abnormal signal was extended as the increasing of dose. However, compared with the 6 months, no significant relieve or advancement was detected.Conclusions: 1.The establishment of rabbit model of TMJ with radiotherapy can be employed as an ideal model for the research of radiation-induced damage in human DMJ. 2. The radiotherapy is capable of inducing the degeneration in the early stage, and the change is positively related with the dose and duration of radiotherapy. Thus the radiotherapy of TMJ may be the promising factor of TMJ dysfunction syndrome. 3. MRI maintains solid sensitivity of detecting early degeneration of TMJ with radiotherapy.
