Abstract


Objective: To investigate cerebral aneurysm aneurysm wall pathology change detection matrix metalloproteinases (MMP-2, MMP-9) expression in the cerebral aneurysms, and explore whether there is a correlation between both clinical features. Growth and the cause of the rupture mechanism studies provide clues for the occurrence of cerebral aneurysms. : Experimental group Province Second Hospital neurosurgery by craniotomy and clipping of aneurysm surgery after removal of part of the tumor tissue, 17 cases of to obtain cerebral aneurysms clinical specimens; dissection in patients with intracerebral hematoma in the control group, 15 cases cut the normal superficial temporal artery, and confirmed by the CTA or MRI in patients with non-vascular diseases. Above all specimens made under the premise of ensuring the life safety of surgery and patients. All specimens were in vitro within 24 hours of immersion in 10% formalin solution, upon further pathological examination. Above specimens by general visual inspection, routine HE staining observed by light microscopy and immunohistochemistry of MMP-2, MMP-9 staining, detection of the aneurysm wall and normal blood vessel wall MMP-2, MMP-9 coloring and expression site expression intensity. Lesion characteristics and clinical features, the statistical analysis of the relationship between MMP-2, MMP-9 of the expression of the patient's age, sex, size of the aneurysm and Hunt-Hess grade. Results: 1 naked eye generally observed 15 cases of normal superficial temporal artery all pink and white colors, medium texture, normal elasticity. 17 cases of aneurysm specimens mostly dark purple or purple and yellow, some aneurysm wall. Aneurysm wall specimens uneven, some specimens aneurysm wall was thin top of the tumor; Some aneurysm rupture, some only a thin layer of transparent tissue left. HE staining under light microscope the structure of each of the normal control group were normal, and no significant inflammatory cell infiltration. Most of the experimental group aneurysm wall has lost its normal three-tier structure: endometrial thinning or remaining layer of endothelial cells, and some even disappear completely. Medial smooth muscle layer atrophy, thinning, fill a large number of fibroblasts, some visible myxoid degeneration. The adventitia organization loosely arranged. A majority of the tumor cavity visible attached thrombosis ingredients. 3 the results of immunohistochemical staining of MMP-2, MMP-9 positive expression of cytoplasmic brownish yellow or dark brown. The vessel wall of the normal control group no significant MMP-2, MMP-9 expression. The aneurysm wall within the experimental group, the adventitia were positive expression of MMP-9 outside the group, in the membrane expression of strength is higher than endometrial MMP-2 group, no difference in the expression of the outer membrane. Expression site is mainly concentrated in the area of ??endothelial cells, macrophages, smooth muscle cells and inflammatory cells. Experimental group, MMP-2, MMP-9 positive expression rates were 88.24% (15/17) and 6.67% (1/15), the positive expression rate of the normal control group were 82.35% (14/17) 13.33% (2/15). Cerebral aneurysm aneurysm wall of the expression of MMP-2, MMP-9 positive expression rate was significantly higher than the normal artery wall. Part of the aneurysm wall is associated with arterial atherosclerosis and collagen chaotically distributed endovascular thrombosis attachment, prompted the formation of cerebral aneurysms may be related to the acquired degenerative disease of the blood vessel wall. 2 MMP-2, MMP-9 were positive expression in the aneurysm wall layers, mainly in the film, the outer membrane. MMP-2, MMP-9 positive expression rate in the aneurysm wall was significantly higher than in normal brain artery group, indicating that they may be involved in the formation and rupture of intracranial aneurysms. 3 MMP-2, MMP-9 in the pathogenesis of cerebral aneurysms plays an important role in their high expression inhibition may effectively reduce the occurrence and development of cerebral aneurysms and provide a new direction for the treatment of cerebral aneurysms.
