Abstract


Objective Hirschsprung (HD) margin expansion section ganglion cells cathepsin D (CAD) and S-100 protein were measured by immunohistochemistry and prognosis . Methods for nearly ten years in our hospital via anal megacolon radical surgery were followed up 111 cases , 74 cases were followed up from the selected 32 cases are grouped. Including 20 males and 12 females . The youngest patient was 44 days , up to 7 years old. 32 cases were confirmed by preoperative barium enema and anal manometry diagnosis, patients with pathologically confirmed HD. Symptomatic group (A group ) , 22 cases of postoperative recurrent diarrhea, bloating, constipation , fever, enteritis , obstruction and other symptoms , which require enemas , antibiotics, rehydration therapy. Asymptomatic group (B group ) 10 cases , surgery may be normal self soon stool without colitis, constipation occurs . Another from the line transverse or sigmoid colostomy and pathological examination of the children selected five cases , the pathological visible mature ganglion cells , as normal control group (C group ) . Pathological specimens for three groups of patients for CAD ganglion cells and S-100 immunohistochemical determination to observe the A, B group intestinal stenosis and dilated segment and C CAD intestinal nerve tissue and the expression of S-100 features integrated postoperative follow-up to analyze the situation . Results A group expansion section on intestinal nerve plexus ganglion cells most CAD weakly positive staining , a few expressed as positive, negative ; B Group C Group expansion section and intestinal myenteric and submucosal plexus most visible positive staining CAD ganglion cells , the individual weakly positive . Most cases of group A divergent section S-100 staining can be seen in a small number of cell -like \plexus cells seen in normal distribution -like \A, B two lateral edge CAD recent expansion section and S-100 expression difference was statistically significant . Conclusion CAD ​​and S-100 is the intestinal ganglion cells highly specific markers could be used as an important means of neurodevelopmental HD . Proximal intestinal segments for expansion joint detection of CAD and S-100, judgment ganglion cells and nerve fibers express will help guide clinical surgical resection boundaries determine the expansion , thereby reducing postoperative colitis, constipation and other complications.
