Objective To study in patients with chronic hepatitis B frequency of NK cells, activation and functional characteristics, and serum transaminase levels, the correlation of viral load; according to liver inflammation and pathological grading, analysis of NK cells in peripheral blood and liver tissue frequency and activation features to further explore the immunological characteristics of NK cells in patients with chronic hepatitis B and its mechanism of action mediated immune injury of the liver. Methodology The study looked at 34 patients with HBeAg-positive chronic hepatitis B patients, 26 patients with chronic HBV carriers and 10 healthy controls were frequency and activity of NK cells, of which 24 cases of HBV infection in patients with liver biopsy. Of the peripheral blood biochemical and viral indicators (alanine aminotransferase levels and serum HBV load), flow cytometry the frequency of peripheral blood NK cells, activation of the receptor expression of CD69; NK cells secrete cytokines IFN-gamma and TNF-alpha and cytotoxic factors perforin and CD107. Immunohistochemical method to detect liver tissue immune cells (CD3 T cells and NK cells), as well as cytokines interferon-gamma and CD107a expression. 1 with chronic HBV carriers and healthy contrast, HBeAg-positive chronic hepatitis B patients with peripheral blood NK cell frequency was significantly reduced, but the expression of CD69 early activation receptor levels, and serum ALT levels were positively correlated (r = 0.402 , p = 0.018) to prompt mature activation of NK cells may be involved in the immune injury of the liver. NK cell function in vitro detection prompts expression of IFN-gamma and TNF-alpha increase of NK cells in patients with chronic hepatitis B in the stimulation of IL-12 or PMA / ionomycin. In patients with chronic hepatitis B NK cells express perforin and CD107a than chronic HBV carriers, and the expression of CD107a levels of serum ALT levels were positively correlated. According to liver inflammation grade, G of light degree of inflammation the G 1-2 group and the degree of inflammation 3-4 group: Show of G 3-4 group of patients the frequency of peripheral blood NK cells decreased expression of CD69 increased. NK cells in vitro function tests prompt in the IL-12 stimulation or PMA / ionomycin, the degree of inflammation weight G 3-4 in patients with peripheral blood NK cells expressing IFN-gamma and TNF-alpha increased and expression of perforin and CD107a is also higher than the G 1-2 patients. According to liver inflammation grade, G of light degree of inflammation the G 1-2 group and the degree of inflammation 3-4 group: find the degree of inflammation G 3-4 liver tissue periportal and lobular area CD57 sup> NK cell number more than the inflammation grade G 1-2 of patients. The the inflammation grade G 3-4 of the patients express IFN-gamma significantly higher than that the inflammation grade G 1-2 of the patients, patients with different degree of inflammation of the liver CD107a expression was no significant difference . Conclusions in patients with chronic hepatitis B its frequency and activation of NK cells (activated receptors, cytokines, and increased expression of cytotoxic factor), liver tissue NK cell aggregation and inflammation of the liver damage is closely related.
