Abstract


Objective: On the basis of conduct the TCM Congestive Heart Failure Syndrome Scale:⑴revise and improve the primary scale of symptoms of congestive heart failure, and constitute a Dialectical standards of primary Congestive Heart Failure.⑵Evaluation the reliability and validity of the new established scale, so as to provide a theoretical basis for the establishment of congestive heart failure Clinical Evaluation System of Traditional Chinese Medicine which integrate Chinese characteristics and integration with modern medicine standards.Methods:⑴Firstly, test a small scale samples of the primary congestive heart failure scale, find and amend the entrys which ambiguous and dissatisfy the practical application, and then form a congestive Heart Failure Syndrome Scale.Some patients were randomly selected by the SF-36 survey of Chinese version,Select 243 hospitalized heart failure patients with heart failure questionnaire survey,simultaneously, randomly select some patients by using the Chinese version of SF-36 quality of life questionnaire survey. At the same time, select 80 healthy volunteers whom after medical in our hospital medical centers as control group to investigate by using congestive heart failure TCM scale.After collection, sorting data, assess the reliability and validity.Reliability assessment, including split-half reliability and internal consistency reliability; validity assessment, including content validity, discriminant validity and criteria validity.⑵data collection: Summary data and establish database by Microsoft EXCEL.⑶statistical analysis: Assess the new scale validity and reliability by using SPSS13.0 and LISERL8.7.Results:⑴the feasibility of scale: In investigation, the recovery and completion rate was 100%, the average completed time is under five minutes, and the feasibility is good.⑵scale reliability: The Cronbach’sa of each dimension for Congestive heart failure Syndrome Scale were:The main symptoms dimension 0.59, head and body dimensions 0.54, emotional dimension 0.31, cold and heat dimension 0.52, food dimension 0.58, sweating dimensions 0.69, sleep dimension 0.56, urine and stool dimensions 0.50, signs dimension 0.71. The scale’s split-half reliability coefficient was: 0.52, total Cronbach’sa coefficient is 0.640.Beside the emotional dimensions and Cronbach coefficient of cold and heat were lower, the other dimensions of Cronbach coefficient were no less than 0.5. Therefore, with statistical significance, the overall reliability is good.⑶scale validity:①content validity: After the four diagnostic methods in data collection, establishment of TCM, K-means clustering and principal component analysis, interviews, expert comment, discussion and repeated measurement test, review and revise, the Congestive heart failure Syndrome Scale was accomplished. It has good representative and content validity.②discriminant validity: Test health people and heart failure patients by T test for each dimension. All dimension P were less than 0.05, except the emotional dimension. The difference between them was significant, since the scale could distinguish the congestive heart failure patients from health people, moreover, the scale has good discriminant validity.③criterion validity :Calculate the correlation coefficients of each dimension about Congestive heart failure Syndrome Scale and the SF-36 quality of life scale. Emotional dimensions of which scale with the SF-36 relative to the dimensions of the correlation coefficient were between 0.29 and 0.53; cold and heat dimension and SF-36 scale in each dimension of the correlation coefficient were between 0.32 and 0.59; The most of other dimensions and the dimensions of SF-36 scales of the correlation coefficient were above 0.5.The correlation of Congestive heart failure Syndrome Scale and the SF-36 quality of life scale is moderate.Conclusion: The overall reliability and validity of the scale is good, could be initially tested in clinical studies
