Abstract

[Objective]To study the therapeutic effect of nitroglycerol on severe acute pancreatitis (SAP) and the underlying mechanisms by investigating the effects of nitroglycerol on the changes in the pathological morphology and CD34 expression of pancreatic tissue and the contents of plasma amylase, IL-1, IL-6, endothelin (ET) and nitric oxide (NO) in SAP rabbit model.[Methods]Healthy male New Zealand rabbit were divided 3 groups, control group, after sham operation 4 h, 150 ml of 0.9% NaCl was injected from ear vein at 2 ml/min; SAP group, after SAP 4 h, 150 ml of 0.9% NaCl was injected from ear vein at 2 ml/min; and Nitroglycerol-treated group, after SAP 4 h, 150 ml nitroglycerol (2μg/kg) was injected from ear vein at 2 ml/min. After SAP 8 h, 16 h and 24 h, blood was gathered. After SAP 24 h, pancreatic tissue was gathered.Serum amylase, IL-1, and IL-6 were detected by full-automatic biochemistry; Serum ET was analyzed by radioimmunoassay; Serum NO was measured by nitric acid reductase. CD34 expression was detected by immunohistochemistry. The pathological morphologic alteration was analyzed by hematoxylin-eosin staining.[Results] 1. After SAP 8 h, 6 h, and 24 h, the levels of serum amylase were significantly increase; however, treatment with nitroglycerol attenuated the increased serum amylase in 8h- 16h-and 24h-SAP, which indicated that nitroglycerol inhibits SAP-induced the release of serum amylase.2. After SAP 8 h, 6 h, and 24 h, the levels of serum IL-1 and IL-6 were increase; however, treatment with nitroglycerol reduced the increases in IL-1 and IL-6 caused by SAP at 8 h, 16 h, and 24 h, suggesting that nitroglycerol attenuates SAP-triggered inflammatory reaction.3. After SAP 24 h, the score of pathological morphologic alteration in pancreas was increase; however, treatment with nitroglycerol reduced the increases in the score of pathological morphologic alteration, which implied that nitroglycerol attenuates the pathological morphologic alteration in pancreas caused by SAP.4. After SAP 8 h, 16 h, and 24 h, the ratios of ET/NO were increase; however, treatment with nitroglycerol decreased the increase in the ratios of ET/NO induced by SAP at 8 h, 16 h, and 24 h, suggesting that nitroglycerol ameliorates the vascular function in SAP.5. After 24 h SAP, the expression of CD34 and microvessel density in pancreas were decrease; however, treatment with nitroglycerol increase the expression of CD34 and microvessel density in pancreas, which indicated that nitroglycerol ameliorates the minicirculation of pancreas after SAP.[Conclusions]1. Nitroglycerol reduced the level of serum amylase, IL-1、IL-6 and the score of pathological morphologic alteration in pancreas of SAP, which indicated that nitroglycerol attenuates the inflammatory reaction and the pathological lesion in pancreas of SAP. 2. Nitroglycerol dereased the ratio of ET/NO and inreased the expression of CD34 and microvessel density in pancreas, which indicated that nitroglycerol attenuates the pathological lesion in pancreas of SAP through ameliorating the vascular function and minicirculation.
