Objectives:Chronic urticaria(CU) is a common skin disorder.Its’ pathogenesis remains unclear.The treatment is facing a very difficult problem.The study indicates that chronic urticaria has relations with autoimmune、gene、environment and some other factors.All the studies indicate the existence of Th/Th2 imbalance with chronic urticaria.At present,many researchers judge the amount of the Th1/Th2 indirectly by detecting the content ofγ-interferon(IFN-γ) or interleukin-4(IL-4) both in basic rsearch and clinical researches.However,the studies at home and abroad are not exactly the same and can not explain all the phenomena about CU.Recently,Th17 is a newly discovered cell subtypes.Th17 subtype model is the most important addition to Th1/Th2 model.Many studies shows that Th17 may play a significant role in many autoimmune diseases and inflammatory diseases.With the discovery of Th17,it will bring a new understanding of the pathogenesis and treatment strategy to chronic urticaria.IL-17 and IL-23 play a very important role in many autoimmune diseases and inflammatory diseases,as two important cytokines of Th17 cells.With the detection of Th17 level in CU,Our purpose is to evaluate the relations between the expression level of IL-17 and IL-23 and CU and the possible immune mechanism of Th17 in the pathogenesis,from the point of view of the imbalance among Th cells.All these may bring new theoretical basis and treatment strategies and new ideas for the immunotherapy of chronic urticaria.Methods:45 CU patients and 42 normal controls were selected.Serum levels of IL-4,IFN-γ, IL-17,IL-23 were determined by ELISA.All data were statistically analyzed.Results:The serum level of IL-4、IL-17、IL-23 in CU patients was significantly increased compared to normal controls,and IFN-γwas significantly decreased.In CU patients, there was no significant correlation between the serum level of IL-4、IFN-γ、IL-17 and IL-23.Conclusion:There exists immune dysfunction and Th differentiation imbalance in CU.Th2 subsets and Th17 subsets increased expression,Th1 subsets are decreased expression. IL-17、IL-23 associated with Th17 cells in chronic urticaria may play an important role in the pathogenesis of CU.The regulation of that functional disorders in the lymphocyte subsets of CU patients and Immune treatment about Th17 and IL-17、IL-23 may be one of the new treatment strategy.
