Objective: To study periconceptional exposure to atmospheric pollutants carbon monoxide gas (carbon monoxide, CO) and the correlation between the first trimester missed abortion. Methods: A retrospective case-control study, to study the relationship of Tianjin atmospheric pollutants CO exposure in the first trimester missed abortion. Monthly average concentration of carbon monoxide in the 2001-2007 six administrative districts of the city's state-controlled monitoring points in March, June, September, December atmospheric obtained from the Tianjin Municipal Environmental Monitoring Center. General hospital in Tianjin city's six districts 15 and obstetrics and gynecology specialist hospital family planning clinics from January 2001 to August 2007 diagnosis of patients with missed abortion case group (n = 975 cases), with the same period the hospital 1:1 ratio by age, randomly selected normal pregnancy line abortion as the control group. SPSS 11.0 software for statistical analysis, t test, rank sum test, chi-square test and other methods to analyze different years, in different months, the heating and non-heating the atmosphere of carbon monoxide pollution levels if there is a difference. Analyzes of missed abortion cases, the number of cases of missed abortion accounts for the month total number of abortions percentage change in trend. Between cases and control groups in January before pregnancy, month of pregnancy, after pregnancy in January of carbon monoxide exposure levels whether there is a difference. The chi-square test analysis exposed to different levels of carbon monoxide, whether the risk of missed abortion there is a difference. Non-conditional logistics regression model for multivariate analysis to explore the age, the relationship between the natural frequency of abortion, gravidity, parity and periconceptional carbon monoxide exposure levels and other factors, with the first trimester missed abortion. Results: 1. Tianjin in June, September and December of carbon monoxide in the atmosphere monthly average concentration statistically significant, the highest in December. Heating period, the average concentration was significantly higher than the non-heating period (P = 0.043). Heating period, missed abortion of all abortion percentage is higher than the non-heating period (P = 0.000) analysis of the Tianjin city's six districts missed abortion patients, found that patients with 20 years -29 years of age (59% ). Of these, 2.6% in patients with previous history of spontaneous abortion. 3 cases of pregnant after a month of CO exposure levels higher than control group (P = 0.041). Month before pregnancy (P = 0.147), gestational month (P = 0.699), the two groups found no exposure levels are different. One month after pregnancy in accordance with the different levels of exposure were analyzed and found the low exposure group (0-1.3mg/m3) and in the high exposure group (1.6-2.1 mg/m3) (P = 0.037), low exposure group and the high exposure group (≥ 2.1 mg/m3) (P = 0.027) were statistically the difference. The relative risk for high exposure group OR = 1.609 (95% CI = 1.029-2.516), the risk of high exposure group is 1.609 times the low exposure group. High exposure group OR = 1.662 (95% CI = 1.059-2.608), high exposure group risk is low exposure group 1.662 times the risk. That pregnant one month after exposure to higher concentrations of carbon monoxide, may increase the risk of missed abortion occurs. 4 non-conditional logistic regression analysis may be other factors associated with missed abortion, found that Mother's age (P = 0.000,0 R = 1.646), the number of spontaneous abortion (P = 0.035, R = 9.370), the pregnant one month after carbon monoxide exposure level (P = 0.038, OR = 1.185) risk factors for missed abortion. Production (P = 0.000,0 R = 0.299) protective factors for missed abortion. The regression equation was Y = -1.784 0.499 X1 2.237X2 0.170X3-1.208X4. Conclusion: one month pregnant women pregnant after exposure to higher concentrations of carbon monoxide, may increase the risk of missed abortion. The higher the exposure concentration, the greater the risk of missed abortion. 2 missed abortion occurred and women ages, spontaneous abortion occurrences relevant. The older, the more the number of previous spontaneous abortion, the higher the relative risk of the occurrence of missed abortion.
