Objective To investigate the bilateral craniectomy craniotomy value in the treatment of bilateral asymmetry of severe traumatic brain injury . Methods February 2005 to December 2008 , 52 cases of the bilateral asymmetry neurosurgery hospital ninth Affiliated Hospital of Guangxi Medical severe traumatic brain injury patients , all with severe head injury were randomly divided into a treatment group and a control group treated patients with bilateral regulation craniotomy craniectomy surgery , unilateral standard large trauma surgical treatment control group , and 3 months after the GOS assessed using statistical methods to analyze the results of the two groups . The treatment group of 24 cases, good recovery in 9 cases , moderate disability cases , severe disability three cases , the plants survive the three cases , 4 deaths . Control group, 28 cases , good recovery in four cases , moderate disability in three cases , severe disability in 5 cases, the plant survive four cases , 12 patients died . Statistical analysis showed encephalocele in the treatment group, intraoperative and postoperative incisional hernia incidence was significantly lower than the control group ( P lt; 0.05 ) , the mortality rate of the treatment group than the control group ( P lt; 0.05 ) , good recovery rate is better than the control group (P lt; 0.05). Conclusion bilateral asymmetry in the treatment of severe traumatic brain injury , bilateral regulative go craniotomy decompression surgery than the traditional one-sided craniotomy to large trauma surgery can be more effective in reducing acute brain surgery bulging and the incidence of postoperative incisional hernia , significantly improve the prognosis and reduce mortality .
