Objective:Evaluate the incidence of vascular cognitive impairment (VCI) in the inpatients with hypertension and its risk factors.Methods:Selected 190 hospitalized patients diagnosed hypertension from Department of Neurology and Cardiology in the North hospital of Second Affiliated Hospital of Dalian Medical University, from September 2009 to December 2010 . According to the score of Montreal Cognitive Assessment (MoCA)Beijing Version, 108 cases were divided into VCI group and 82 cases into without VCI group.1. To explore the incidence of VCI in the inpatients with hypertension and the incidence of VCI among different age groups. The number of risk factors for the extent of occurrence of VCI.2.To assess the effects of gender, age, body mass index, education level, smoking, extracarotid atherosclerosis, diabetes, high cholesterol, coronary heart disease, atrial fibrillation, cerebral infarction, recurrent TIA 12 factors for VCI in the inpatients with hypertention,using a single factor analysis. To identify the factors mentioned above if there were statistical differences between the two groups.3. the statistically significant risk factors mentioned above were introduce into an unconditional logistic regression analysis model, where the variables were selected by backward procedure.4. The systolic pressure, diastolic blood pressure introduced into the linear regression model, to MoCA score as dependent variable linear regression .To evaluate the relation between severity of VCI and blood pressure levelResults:1. the incidence of VCI1) Of the 190 inpatients with hypertention ,the incidence of VCI in the inpatients with hypertension is 56.8%. 2) In the 45-54 age group, the incidence of VCI was 31.9%; 55-64 age group, the incidence of VCI is 40%; 65-74 age group, the incidence of VCI is 71.2%; 75-85 age group, the incidence of VCI is 91.7%.3) If participants having one risk factors, the incidence of VCI is 56.8%;if having two risk factors ,it is 78.2% ,79.7% for having three risk factors and 89.6% for having all four risk factors.2. Single factor analysis:There were significant differences in age, education level, extracarotid atherosclerosis, diabetes, hypercholesterolemia and cerebral infarction (P<0.01 or P<0.05)between VCI group and without VCI group.3. Logistic regression analysis:After adjustment for confounding with a model for multiple regression analysis, age (OR=1.054; CI:1.027-1.083; p < 0.001), education level (OR=1.054; CI:1.027-1.083; p < 0.001)and cerebral infarction (OR=1.054; CI:1.027-1.083; p < 0.001) were found to be independent risk factors for VCI.4. Linear regression:The severity of VCI in inpatients with hypertension was correlated with systolic blood pressure.Conclusion:1. The incidence of VCI in the inpatients with hypertension is 56.8%. The more risk factors inpatients of hypertension have, the higher risk of VCI will be.2. Age, low education levels, and cerebral infarction in inpatients with hypertention were found to be independent risk factors for VCI. With increasing age, risk of the VCI increases.3. The severity of VCI in inpatients with hypertension was correlated with systolic blood pressure.
