Objective To evaluate of common allergens atopic patch test (atopy patch test, APT) of atopic dermatitis patients diagnostic significance, and to explore the role of dust mite allergens in the pathogenesis of atopic dermatitis. Methods common inhalant allergens (house dust mites, dust mite, cat epithelium, dog epithelium and Indus pollen) and ingested allergens (egg white, milk, soybeans, wheat and shrimp) in 79 cases of atopic dermatitis patients in special should patch test, while the corresponding allergen skin prick tests and specific IgE evaluation indicators of sensitivity, specificity, and positive predictive value of APT; while further analysis of household dust mites and dust mites APT results and atopic dermatitis clinical phenotype and the relationship of the disease. Results of common inhaled allergens in house dust mites and dust mite APT positive rate highest were 25.3% and 29.1%, cat epithelium, dog epithelium and the Indus pollen APT positive rate of 13.9%, 15.2% and 8.9%, respectively; common food allergens in soybeans APT positive rate of 26.7%, the highest, egg white, milk, shrimp and wheat APT-positive rates were 23.7%, 23.4%, 23.1% and 16.0%. SIgE as the gold standard to analyze the sensitivity and specificity of the APT, APT diagnosis of 5 kinds of inhalant allergens (85.2% to 92.6%) and high specificity, low sensitivity (11.1% to 32.3%), the diagnosis of households dust mites and dust mite positive predictive value of 95.0% and 91.3%, respectively. With allergic rhinitis and non-allergic rhinitis patients with AD, household dust mites and dust mite APT-positive rate differences were not statistically significant. Endogenous the AD and exogenous AD, household dust mites and dust mite APT positive rate differences were not statistically significant. The positive rate of household dust mites and dust mite APT with dry skin severity score was no correlation. The increased allergen concentrations doubled, household dust mites and dust mite APT-positive rate increased, but not statistically significant; changing APT matrix (increase emulsifier) ??after house dust mite APT positive rate increased to 54.2% from 25.3%, and statistically significant, the dust mites APT positive rate also increased, but not statistically significant. Conclusion dust mite atopic sex dermatitis in patients with the most common inhalant allergens; of the APT diagnosis inhalation allergen specificity 90%, positive predictive value of the diagnosis of dust mite allergy; merger allergic rhinitis in patients with AD APT dust mite positive rate was significantly increased; the endogenous AD and exogenous AD patients dust mites APT positive rate was no significant difference; APT positive rate of dust mites and AD dry skin severity score was no correlation; allergen protein concentration or change matrix (increase emulsifier), dust mites APT positive rate of increase.
